| Among the many chronic diseases of the elderly,the incidence of chronic cardiovascular diseases is extremely high,due to the long treatment cycle and health care cycle,as well as the high recurrence rate and mortality rate,which seriously affects the physical health and quality of life of the elderly in the community.The traditional cardiovascular disease treatment model focuses on helping elderly patients alleviate pain and prolong life,while ignoring the preventive work of early intervention of cardiovascular disease and patient self-management.Long-term care needs in a home environment are not met.In the era of Mobile Internet,chronic disease management is no longer limited to traditional treatment methods,and more emphasis is placed on the use of information means to achieve chronic disease prevention and self-health management.At the same time,in the community environment,it is necessary for relevant institutions to take joint actions to provide comprehensive medical,health care and nursing services for these elderly patients,and form a smart chronic disease management method that integrates medical care and nursing care.Based on the above social background,this paper will start from the two aspects of chronic disease management and chronic disease prevention,build a smart medical care management model for chronic cardiovascular diseases in the elderly based on data mining,realize the management of smart chronic diseases in community and home environments,and correct the shortcomings of the existing disease management model of "emphasizing treatment over prevention" and "separation of medical care and nursing care".The main research content of the article is as follows:(1)By analyzing the drawbacks of the traditional management model of chronic cardiovascular disease in the elderly,and trying to apply emerging technologies such as the Internet of Things and data mining to the process of disease treatment and maintenance,the integrated management model of medical care for elderly chronic cardiovascular diseases based on the guidance of the ICCC framework is constructed,providing a comprehensive management method for elderly patients with refined medical care and maintenance.At the same time,the service operation framework and specific application scenarios of the medical and nursing care management model of chronic cardiovascular disease in the elderly are analyzed and constructed,so as to provide a feasible method for the rational allocation of resources such as community services and medical services.(2)By constructing a heart health assessment model and risk prediction model based on deep learning,in order to achieve early disease intervention,disease prevention is regarded as the core link of chronic disease management,and through analysis and assessment of patient physical condition and self-service disease risk warning,it provides a basis for individual self-health management and doctor-assisted diagnosis.(3)This paper organically integrates and applies the management concept of smart medical care model and the disease risk prediction method based on data mining,complete the prototype development and implementation of the smart medical care system,promote the construction of the cardiovascular disease specialty medical association and the regional medical community,solve the implementation problems of two-way referral and hierarchical diagnosis and treatment,and promote the construction of the social medical and health service system. |
