| Objective: By observing the clinical effect of Sanxiao Lishui Decoction on edema in patients with membranous nephropathy,to study the effectiveness and safety of treatment plan of edema in membranous nephropathy with deficiency of lung,spleen and kidney combined with water stasis syndrome under the guidance of "theory of water-fluid metabolism of lung,spleen and kidney three organs",to provid some theoretical basis for the treatment of membranous nephropathy edema with traditional Chinese medicine.Methods: 66 clinical cases included in the study were randomly divided into33 cases each in the control group and the experimental group.The control group was given basic western medicine treatment combined with furosemide 20 mg and spironolactone 20 mg orally once a day,and the experimental group was given basic western medicine treatment combined with Sanxiao Lishui Decoction.The treatment course was 3 weeks.Observe and record laboratory indicators such as body weight,24 h urine volume,24 h urine protein,serum albumin,fibrinogen,and serum creatinine in the two groups before and after treatment.Results: 1.There was no statistical difference in the general information,edema degree,laboratory indicators,and TCM syndrome scores between the two groups before treatment(P>0.05),which were comparable.2.Compared with before treatment,the body weight,24 h urine volume,24 h urine protein quantification,and serum albumin of the two groups were improved after treatment(P>0.01);fibrinogen in the experimental group decreased(P>0.01),There was no statistical difference between the control group and before treatment(P > 0.05);after treatment,there was no statistical difference between the two groups in terms of body weight,24 hour urine volume,24 hour urine protein quantification,and serum albumin(P>0.05).There was a statistical difference in fibrinogen(P>0.01),and the test group was significantly better than the control group.3.Compared with before treatment,the TCM syndrome score in the experimental group decreased after treatment,which was statistically significant(P>0.01),and there was no statistical difference in the control group compared with before treatment(P>0.05);after treatment,the two groups There was a statistically significant difference in TCM syndrome scores(P>0.01).4.Efficacy evaluation of edema: the effective rate of the test group was83.33%,and the effective rate of the control group was 43.33%(P>0.01);evaluation of the curative effect of TCM syndrome: the effective rate of the experimental group was 60.00%,and the effective rate of the control group was 20.00%(P>0.01);Disease efficacy evaluation: the effective rate of the test group was 16.67%,and that of the control group was 0.00%(P>0.05).5.Safety evaluation: Compared with before treatment,the serum creatinine in the experimental group decreased(P>0.05),and there was no statistical difference in the control group(P>0.05);after treatment,there was no statistical difference in the serum creatinine levels between the two groups(P > 0.05);Compared with before treatment,blood potassium in the experimental group increased(P>0.01),and blood potassium in the control group decreased(P > 0.01);after treatment,there was a statistical difference in blood potassium levels between the two groups(P>0.01);Hypokalemia occurred in 4 patients in the control group,and no adverse reactions occurred in the test group during treatment;the safety evaluation level of the control group was level 3,and the safety level of the test group was level 1.Conclusions: Sanxiaolishui decoction has a significant effect on membranous nephropathy,lung,spleen,kidney deficiency and water stasis mutual edema,and has a certain effect on improving proteinuria,serum albumin level,renal function,blood hypercoagulability,and can also prevent the occurrence of adverse events.Applying the theory of water-fluid metabolism of lung,spleen and kidney to guide the clinical treatment of edema in membranous nephropathy with deficiency of lung,spleen and kidney combined with water stasis syndrome has good efficacy and safety. |
