Objective:To determine the level of popular fibrosis indicators in polymyositis/dermatomyositis(PM/DM)and systemic scleroderma(SSc)and assess their significance in detecting connective tissue disease-related interstitial lung disease(ILD).Methods:A total of 83 patients with PM/DM(34 cases)and SSc(49 cases)who visited the outpatient or inpatient department of our hospital between January 2016 and Januay 2022 were enrolled.The expression levels of the fibrosis and inflammation indices and related antibodies in the two groups were first analyzed.Thereafter,the correlation between these indices and the degree of lung lesions(lung CT score)in PM/DM and SSc-related ILD was determined.With the lung CT score as the dependent variable and the fibrosis and inflammation indices and related antibodies as the independent variables,multiple linear regression was used to determine the factors influencing the lung CT score.We compared the fibrosis indicators levels between the two groups.Additionally,we analyzed the fibrosis and inflammation indicators levels in patients with PM/DM before and after treatment to further verify the results.Results:1.The type-Ⅳ collagen,laminin,hyaluronic acid,and type-Ⅲprocollagen N-terminal peptide levels in patients with PM/DM were higher than the normal values.Fibronectin was positively correlated with the severity of lung lesions in PM/DM-ILD(r=0.349,P=0.043).In addition,hypersensitive C-reactive protein(hs-CRP)level and erythrocyte sedimentation rate(ESR)were positively correlated with the severity of lung lesions in PM/DM-ILD.ESR,hs-CRP,and anti-Jo1,anti-centromere B,and anti-PM-Scl antibody levels also affected the severity of the PM/DM-ILD lesions.However,fibronectin levels had the greatest impact;the higher the fibronectin levels,the more serious the PM/DM-ILD lesion was.2.The type-Ⅳ collagen,hyaluronic acid,and type-Ⅲ procollagen N-terminal peptide levels in patients with SSc were higher than the normal values.Fibronectin levels were not high.Additionally,laminin was positively correlated with the severity of the SSc-ILD lesion(r=0.524,P<0.001),but did not affect the severity of the SSc-ILD lesions.In addition,the higher the ESR level with no anti-centromere B antibodies,the more serious the SSc-ILD lesion was.3.Fibronectin levels in patients with PM/DM were higher than those in patients with SSc.The relationship between fibronectin and PM/DM was stronger.Conclusion:There is a certain correlation between popular serum fibrosis indicators and CTD-ILD,and some indicators are related to the degree of ILD lesions.Fibronectin levels affect the degree of PM/DM-ILD lesions,while laminin levels are positively correlated with the degree of SSc-ILD lesions.
