| Objective: To observe the clinical efficacy of Xiaoying Zhitong Decoction combined with Xiaoying Zhitong Transdermal Patch in the treatment of subacute thyroiditis of excessive heat toxin type,and to provide new ideas for the treatment of subacute thyroiditis with traditional Chinese medicine.Methods: A total of 90 patients with subacute thyroiditis of heat toxin accumulation type who accorded with the diagnostic criteria in the outpatient and inpatient department of endocrinology of the affiliated hospital of shandong university of traditional Chinese medicine were randomly divided into compare group,Chinese herb treated group and traditional Chinese herb+ external therapy group,with 30 cases in each group.The compare group was cured with ibuprofen sustained release capsule,the Chinese herb group was cured with Xiaoying Zhitong decoction on the basis of the compare group,and the Chinese herb+external theraoy group was cured with Xiaoying Zhitong decoction combined with Xiaoying Zhitong transdermal patch on the basis of the compare group.8 weeks was a therapeutic course,a total of 1 courses of treatment.The changes of fever subsidence time,level of thyroid gland swelling,blood sedimentation rate,thyroxine function,thyroid goiter,Chinese medicine syndrome scores and safety indexes were observed before and after treatment.Data were processed and analyzed by SPSS26.0 and to evaluate the therapeutic effect and security of Xiaoying Zhitong Decoction and Xiaoying Zhitong Transdermal Plaster in the treatment of subacute thyroiditis with excessive heat toxin type.Results: 1.Comparison of clinical efficacy of Western medicine: The total effective rates of Chinese medicine + external application group,Chinese medicine group and control group were 96.43%,89.19% and 77.78%,respectively.The clinical efficacy of the Chinese herb treated group and traditional Chinese herb + external therapy group was superior to the control group(P<0.05),and traditional Chinese herb + external therapy group was better than the Chinese herb treated group(P<0.05).2.Therapeutic effect of syndrome of traditional: The total effective rates of Chinese medicine +external application group,Chinese medicine group and control group were respectively 96.43%,88.89% and 74.07%.Both the traditional Chinese herb +external therapy group and the Chinese herb treated group were superior to the control group(P<0.05),and the traditional Chinese herb + external therapy group had more advantages than the Chinese herb treated group(P<0.05).3.Clinical symptom relief: The thyroid pain relief and fever subsidence time of the traditional Chinese herb + external therapy group and the Chinese herb treated group had more advantages in those of the control group(P<0.05),and the traditional Chinese herb + external therapy group were shorter to relieving pain than the Chinese herb treated group(P<0.05),but there was no significant difference in antipyretic time between the two groups(P>0.05).4.Erythrocyte sedimentation rat: After treatment,the erythrocyte sedimentation rate of the Chinese medicine + external application group,the Chinese medicine group and the control group was improved compared with that before treatment(P<0.05),the Chinese medicine + external application group and the Chinese medicine group were better than the control group(P<0.05),and the Chinese medicine + external application group was better than the Chinese medicine group(P<0.05).5.Thyroid function: After treatment,the thyroid function of the traditional Chinese medicine + external application group,the traditional Chinese medicine group and the control group was improved compared with that before treatment(P<0.05),and the improvement of FT3 in the traditional Chinese medicine + external application group and the traditional Chinese medicine group was better than that in the control group(P<0.05),but there was no significant difference in FT4 and TSH among the three groups after treatment(P>0.05).6.Degree of goiter: The degree of goiter in the Chinese medicine + external application group,the Chinese medicine group and the control group was improved compared with that before treatment(P<0.05).The Chinese medicine + external application group and the Chinese medicine group were significantly better than the control group(P<0.05),and the traditional medicine + external application group improved thyroid enlargement more significantly than the Chinese medicine group(P<0.05).7.TCM syndrome score: The syndrome scores of the Chinese medicine + external application group,the Chinese medicine group and the control group were improved compared with those before treatment(P<0.05).The Chinese medicine + external application group and the Chinese medicine group were better than the control group(P<0.05),and the Chinese medicine +external application group was better than the Chinese medicine group(P<0.05).8.Safety indicators: After treatment,the relevant indicators were normal,there was no difference between the groups,and there was no obvious adverse reaction.Conclusion: Xiaoying Zhitong Decoction combined with Xiaoying Zhitong Transdermal Patch can shorten the fever time and thyroid pain time of patients with subacute thyroiditis of heat toxin accumulation type,improve erythrocyte sedimentation rate,thyroid function and goiter degree,and improve the symptoms of traditional Chinese medicine. |
