| Objective:Chemotherapy-induced Leukopenia(CIL)is a problem that can not be ignored in bone marrow inhibitory chemotherapy.At present,the prevention and treatment of western medicine is limited,and in order to benefit more patients,it is necessary to introduce other effective intervention measures in time.Acupuncture has been used to prevent and treat diseases for thousands of years,and it has also played an active role in the prevention and treatment of CIL.It is an important factor affecting on their compliance that is the comfort of patients during chemotherapy.Through clinical observation,it is found that needle acupuncture is effective in the prevention and treatment of CIL.And the patients showed good acceptance and feeling of needle use.But the existing research data are scarce and lack of statistical analysis.Based on CIL’s literature on acupuncture-related therapy,this study summarizes and analyzes the characteristics of acupoint selection and acupoint prescription rules of acupuncture and moxibustion in the treatment of CIL.After defining the core acupoints,taking the patients with colorectal cancer who received postoperative adjuvant chemotherapy for the first time as the research object.And then explore the clinical efficacy of needle in the prevention and treatment of CIL through a randomized controlled trial.Methods:1.Data mining research on acupuncture and moxibustion treatment of CILThe literatures about acupuncture-related therapy for CIL before January 1,2022 were searched in Chinese and English databases at home and abroad.Qualified documents obtained after screening by the standard of selection.Using Excel form to summarize and sort out these data.Conduct frequency statistics with the help of IBM SPSS Modeler 18.0.Perform the modeling analysis by Apriori Association Rules.Construct the complex network of acupoint co-occurrence core by Cytoscape3.9.1 to extract the acupoint cluster with dense connection.2.Clinical control study of needle acupuncture in prevention and treatment of CILA total of 48 patients with colorectal cancer who received postoperative adjuvant chemotherapy for the first time were randomly divided into conventional group group(n=24)and acupuncture group(n=24).The conventional group(n=24)received CAPEOX chemotherapy and routine care.The acupuncture group(n=24)received needle acupuncture on Zusanli,Sanyinjiao,Dazhui,Shenshu and Xuehai once a day for 3 days prior to chemotherapy,for 5 consecutive days rest for 2 days,a total of 10 days.Both groups underwent a 21-day treatment for a total of one treatment course.Changes in the white blood cellindices on day 1 before chemotherapy and days 7,14,and 21 days after chemotherapy were compared among the groups.Syndrome scores of TCM on day 1 before chemotherapy and days 7,14,and 21 after chemotherapy were observed.Result:1.Data mining research on acupuncture and moxibustion treatment of CILA total of 39 acupoints were used in 131 articles.The total frequency of use is406.The top five acupoints used frequently are Zusanli,Sanyinjiao,Dazhui,Shenshu and Xuehai.Bladder meridian,stomach meridian and spleen meridian are the most frequently used in acupoint meridian tropism.Mainly distributed in the lower limb and back.The analysis of association rules shows that Sanyinjiao-Zusanli and Xuehai-Sanyinjiao-Zusanli have the highest correlation intensity and support.2.Clinical control study of needle acupuncture in prevention and treatment of CIL(1)Compared with those before chemotherapy,the leukocyte levels in both groups decreased on the 7th and 14 th day after chemotherapy(P < 0.01).However,compared with the conventional group,the white blood cell count in the acupuncture group was higher(P < 0.01),indicating that there were significant differences between the two groups,and the acupuncture group was significantly better than the conventional group.There was no significant difference in leukocyte level between the two groups on the first day before chemotherapy and the 21 st day after chemotherapy(P > 0.05).(2)Compared with those before chemotherapy,the scores of TCM syndromes in both groups were higher on the 7th and 14 th day after chemotherapy(P < 0.01).However,the score of TCM syndrome in the acupuncture group was significantly lower than that in the conventional group at two time points(P < 0.05),indicating that the acupuncture group was superior to the conventional group.There was no significant difference in TCM syndrome scores between the two groups before chemotherapy and on the 21 st day after chemotherapy(P > 0.05).Compare with the conventional group,the symptoms of fatigue and dizziness all have obvious improvement on the 7th and 14th(P < 0.01),and 21th(P < 0.05)day after chemotherapy in the acupuncture group.On the 14 th day after chemotherapy,the facial expression of patients was significantly different(P < 0.05),and the symptoms of lack of food and tolerance,nausea and vomiting were significantly relieved at the same time in the acupuncture group(P < 0.01).Conclusion:1.Acupuncture is effective in the prevention and treatment of CIL,and the overall rule of acupoint selection is to warm the kidney and consolidate the foundation,invigorate the spleen and replenish qi,which reflects the clinical treatment principle of CIL.2.Needle acupuncture is an effective and safe treatment modality for improving leukopenia in colorectal cancer patients afterl chemotherapy.Compared with the conventional group,needle acupuncture improved clinical symptoms after chemotherapy more effectively,such as fatigue,anorexia,nausea and vomiting,dizziness,and so on. |
