Objective:This study intends to carry out a randomized controlled clinical study on the treatment of hemiplegic shoulder pain after stroke by thumb-tack needle for subcutaneous embedding combined with PNF on the pain level,inflammatory factors,upper limb function and quality of life of patients with hemiplegic shoulder pain.Methods: A total of 156 stroke patients with hemiplegia and shoulder pain who received treatment in Fengcheng People’s Hospital from December2021 to January 2023 were selected as research objects,and were divided into two groups(52 cases),PNF group(52 cases)and combined treatment group(52 cases)according to random number table method.Patients in the PNF group were treated with a thumb-tack needle for subcutaneous embedding,patients in the PNF group were treated with PNF,and patients in the combined treatment group were treated with PNF combined with a thumb-tack needle for subcutaneous embedding.Visual analog scale(VAS)was used to evaluate the pain level of patients.The upper limb function of the patients was evaluated by the upper limb part of the FMA scale.GQOL-74 was used to evaluate the quality of life of the patients.Inflammatory factors [IL-6 and TNF-α] were detected by ELISA,and the clinical efficacy of the patients was evaluated.Results: 1.Comparison of VAS score and FMA score among the three groups before treatment(P>0.05).Compared with before treatment,VAS score of the three groups decreased(P<0.05)and FMA score increased(P<0.05)after 1 and 2 months of treatment.VAS score of the combined treatment group was lower than that of the PNF group(P<0.05).The FMA score was higher than that in PNF group and thumb-tack needle for subcutaneous embedding for subcutaneous embedding group(P<0.05);2.The levels of IL-6 and TNF-α in the three groups were compared before treatment(P>0.05),and the levels of IL-6 and TNF-α in the three groups were decreased after 1 and 2 months of treatment(P<0.05),and the levels of IL-6 and TNF-α in the combined treatment group were lower than those in the PNF group and the PNF group(P<0.05);3.The scores of physical function,mental function,social function and role function in the three groups before treatment were higher than those before treatment(P>0.05).The scores of physical function,mental function,social function and role function in the combined treatment group were higher than those in the PNF group after treatment(P<0.05);4.The clinical effective rate of combination treatment group was 92.31%(48/52),which was higher than that of PNF group(75.00%(39/52)and PNF group(76.92%(40/52)(P<0.05).Conclusions: 1.PNF combined with thumb-tack needle for subcutaneous embedding can effectively relieve limb pain in stroke patients with hemiplegia;2.PNF combined with thumb-tack needle for subcutaneous embedding can effectively promote the recovery of upper limb function in stroke patients with hemiplegia;3.PNF combined with thumb-tack needle for subcutaneous embedding can effectively improve the quality of life of stroke patients with hemiplegia,and promote the overall efficacy of patients.
