Abstract/Summary:
Objective:To explore the risk factors that affect the rapid decline of forced expiratory volume in the first second(FEV1)of community residents.Methods:From March 2018 to March 2019,895 community residents were enrolled in Hongguang Livable and Linheli communities in Haigang District,Qinhuangdao City,Hebei Province,and a prospective study was started.The general data of the subjects was collectedincludegender,age,waist circumference,hip circumference,waist-hip ratio and smoking history.Detection of lung function related indicators:the first FEV1test was conducted between March 2018 and March 2019;Related laboratory indexes:interleukin-6(IL-6),eosinophil percentage(EOS%)and blood glucose(BG);1 year later,the second FEV1test was conducted to calculate the annual decline of FEV1.According to the decline rate of FEV1,the research objects were divided into experimental group with rapid decline(annual decline of FEV1≥30ml)and control group with non-rapid decline(annual decline of FEV1<30ml).The differences of observation indexes between the two groups were compared,and the risk factors leading to the rapid decline of FEV1were further clarified by binary Logistic regression analysis.Results:A total of 320 subjects were finally included,including209 cases in the experimental group and 111 cases in the control group.The results showed that there was no significant difference in sex,age,smoking history and blood glucose between the two groups(P=0.548;0.521;0.459;0.351);Waist circumference was significantly lower than that of the control group,and the difference was statistically significant(84.71±9.96cm vs 90.66±10.72cm,P<0.001).Hip circumference was significantly lower than that of the control group,and the difference was statistically significant[96.00(92.00,104.00)cm vs 102.50(96.00,109.00)cm,P<0.001].The waist-hip ratio was significantly lower than that of the control group,and the difference was statistically significant(0.88±0.07 vs 0.89±0.10,P<0.001).The first FEV1was significantly higher than that of the control group,and the difference was statistically significant(3.40±0.56L VS 2.66±0.55 L,P<0.001).The second FEV1was significantly higher than that of the control group,and the difference was statistically significant(3.35±0.56L vs 2.65±0.55L,p<0.001).△FEV1(the difference between the first FEV1and the second FEV1)was significantly higher than that of the control group,and the difference was statistically significant(54.71±16.99ml vs 12.11±5.80ml,p<0.001);The level of interleukin-6 was significantly higher than that of the control group,and the difference was statistically significant[1.87(1.16,3.05)ng/L vs 1.45(1.03,2.07)ng/L,P<0.001];The percentage of eosinophils was significantly higher than that of the control group,and the difference was statistically significant(2.66±1.55%vs 2.32±1.35%,P<0.001).The results of Logistic regression analysis showed that first FEV1,IL-6 and EOS%were the independent risk factors leading to the rapid decline of forced expiratory volume in the first second(OR=9.475,P<0.001;OR=1.391,P<0.001;OR=1.161,P<0.001).Conclusion:Among community residents,first FEV1,IL-6 and EOS%are independent risk factors for rapid decline of FEV1.
