Objective: To compare the characteristics of intestinal flora between elderly patients with sepsis and healthy elderly people, and to explore the correlation between the changes of intestinal flora and the levels of inflammatory factors in elderly patients with sepsis. Methods: The blood and feces of 14 elderly patients with sepsis(observation group)and 30 normal elderly people in the community(control group)from September 2020 to March 2021 were collected. The inflammatory markers(interleukin-6(IL-6),C-reactive protein(CRP),heparin-binding protein(HBP))and fecal flora components of the two groups were measured respectively. The diversity and structure of the flora in the two groups were analyzed, and the correlation between the changes of intestinal flora and the levels of inflammatory factors in the two groups was analyzed. Results: There was no significant difference in age and gender between the two groups(P > 0.05).There were significant differences in the levels of inflammatory factors between the two groups(P < 0.05).The Shannon,Simpson and Pielou indexes of intestinal flora αdiversity in the observation group were lower than those in the control group, and the difference was statistically significant(P < 0.05).There was no significant difference in Chao index between the two groups(P > 0.05).There were some marker strains in each group, such as Bacillus,Lactobacillus...in the observation group,Lachnospira FCS020,Erysipelothrix UCG_003...in the control group.Compared with the species differences of the intestinal flora in the two groups,at the genus level, the relative abundance of the genus Eubacterium mucilaginosus,Rothia, and Dalma...in the observation group decreased, and Enterococcus,Ricinus, and Sutter ’s bacteria...are relatively enriched;there was a significant correlation between Shannon index and CRP level in the observation group(R = 0.547,P <0.05),and there was a significant correlation between Chao index and CRP level(R = 0.546,P < 0.05).There was no statistical correlation between the remaining alpha diversity index and inflammatory factors;there was a correlation between the marker bacteria and the level of inflammatory factors in the observation group. For example, the unclassified Clostridium60 group bacteria were significantly correlated with the level of IL-6 at the level of order, family and genus,R value = 0.678,P < 0.01.Conclusion: The diversity and evenness of intestinal flora in elderly patients with sepsis were lower than those in healthy elderly people. The changes of intestinal flora in elderly patients with sepsis were correlated with the levels of some inflammatory factors.
