Objective: This study aims to observe the therapeutic effect of Danshen Huayu Tang combined with Western medicine on lower limb arteriosclerosis obliterans(ASO)after4 weeks and before treatment,and explore the potential mechanism of Danshen Huayu Tang in treating ASO using network pharmacology technology,providing scientific basis for clinical treatment and experimental ideas for the future.Materials and methods:1.This study used a random number table method to divide patients who met the inclusion and exclusion criteria into a control group and an observation group.The general information of the patients,ankle brachial index,transcutaneous partial pressure of oxygen,vascular endothelial growth factor,serum C-reactive protein,skin temperature,color,limb pain,distance and time of intermittent claudication were recorded and statistically analyzed.Finally,the efficacy was summarized and calculated,The data was statistically analyzed using SPSS 26.0.2.Log in to TCMSP to obtain the effective chemical components of Danshen Huayu Tang and its targets for action on the human body.Obtain disease targets related to lower limb arteriosclerosis obliterans through Gene Cards,OMIM,pharm Gkb,TTD,and Drug Bank databases.Screen for common targets and obtain the relationship between relevant therapeutic targets.Then,conduct GO and KEGG enrichment analysis using R language.Result:1.A total of 62 cases were enrolled in this study.There are 44 males and 18 females,31 in the control group and 31 in the observation group.Among them,1 case was terminated in the control group and 2 cases in the observation group.There were 30 cases in the control group and 29 cases in the observation group.The age and gender of the two groups of patients were tested by t-test,chi square test,etc.,with a P>0.05,and there was no statistically significant difference between the two groups,indicating comparability.The age,gender,VEGF comparison,and CRP comparison between the two groups of patients were analyzed by t-test and chi square test,with P>0.05.The differences between the two groups were not statistically significant and were comparable.2.Clinical efficacy analysis:(1)The ABI of the two groups before treatment was compared by t-test,P>0.05.After treatment,the ABI of both groups improved compared to before,and the ABI in the observation group increased significantly compared to the control group.After testing,P<0.05,the difference was statistically significant,and the observation group was better than the control group.(2)Both groups of treatment had an increasing effect on transcutaneous oxygen partial pressure.Before treatment,t-test showed no statistically significant difference(P>0.05),indicating comparability.There was a significant difference(P<0.05)between the two groups after treatment,and the observation group was superior to the control group.(3)Both groups showed an elevated effect on VEGF,with the control group showing less significant increase in VEGF compared to the observation group.After statistical analysis,P<0.05,indicating a statistically significant difference(4)The CRP levels before treatment in both groups were higher than the median concentration in healthy adults,but after t-test,the difference in CRP levels between the two groups was not statistically significant(P>0.05).After treatment,the difference between the two groups was statistically significant(P<0.05)after t-test,and the observation group was slightly better than the control group.(5)The comparison of skin temperature,color,limb pain,distance and time of intermittent claudication between the two groups before and after treatment showed a statistically significant difference(P<0.05)through t-test.Among the 30 cases in the control group,11 were significantly effective,14 were effective,and 5 were ineffective,with a total effective rate of 83.33%;Among the 29 cases in the observation group,17 were significantly effective,11 were effective,and 1 was ineffective,with a total effective rate of 96.55%.After rank sum test,the difference was statistically significant(P<0.05).The observation group showed better efficacy in improving ASO symptoms than the control group.3.The key chemical components of Danshen Huayu Tang in the treatment of ASO were selected,including quercetin,luteolin,kaempferol,tanshinone IIa,houttuynia cordata,etc.,totaling 135,and 164 drug disease common target genes;GO enrichment analysis results include reactions to heterologous organisms,stimulation reactions,membrane rafts,membrane microregions,nuclear receptor activities,ligand activated transcription factor activities,etc.KEGG enrichment analysis results include lipid and atherosclerosis pathways,PI3K-Akt signaling pathway,MAPK signaling pathway,etc.Conclusion:(1)The combination of Danshen Huayu Tang and Western medicine basic treatment is superior to simple Western medicine basic treatment in terms of increasing VEGF and reducing CRP.(2)The combination of Danshen Huayu Tang and basic Western medicine treatment has better clinical efficacy in improving skin temperature,color,limb pain,transcutaneous partial pressure of oxygen,ABI,distance and time of intermittent claudication compared to simple basic Western medicine treatment.(3)Through network pharmacology research,it is shown that Danshen Huayu Decoction can treat ASO through the complex mechanism of multiple components corresponding to multiple targets acting on multiple pathways,specifically,the effective components in Danshen Huayu Decoction,such as quercetin,luteolin,kaempferol,tanshinone IIa,houttuyfon,etc.,through MAPK signaling pathway,lipid and atherosclerosis pathways,AGE-RAGE signaling pathway in diabetes complications PI3 K Akt signaling pathway,fluid shear stress and atherosclerosis pathway can enhance anti injury ability,regulate lipid metabolism,inhibit angiogenesis,inhibit inflammatory response,inhibit oxidative stress response,regulate cell apoptosis and inhibit cell proliferation,and further play a role in treating ASO.
