| Purpose:To explore the influence of modified Zhengan Xifeng Decoction on ambulatory blood pressure,blood lipid,angiotensin Ⅱ(Ang Ⅱ),and traditional Chinese medicine(TCM)syndromes of patients with primary hypertension of Yin deficiency and Yang hyperactivity syndrome,to determine its clinical efficacy.Methods:A total of 70 patients with primary hypertension who met the inclusion criteria in the Department 4 of cardiovascular medicine of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from January 2022 to November 2022 were divided into the treatment group and the control group according to the random number table,with 35 cases in each group.The control group was treated with the valsartan capsule,while the treatment group was additionally treated with modified Zhengan Xifeng Decoction.The treatment course is three weeks.The changes in ambulatory blood pressure,angiotensin Ⅱ(Ang Ⅱ),blood lipid,and TCM syndrome scores before and after treatment were observed,and statistical analysis of each index was conducted.Result:1.Comparison of curative effects on ambulatory blood pressure,change of blood pressure rhythm,and total effective rate of blood pressure control in patients:After treatment,24-hour mean systolic blood pressure and 24-hour mean diastolic blood pressure,day mean systolic blood pressure and day mean diastolic blood pressure,night mean systolic blood pressure and night mean diastolic blood pressure of patients in the two groups all decreased(P < 0.05),and the treatment group was superior to the control group(P < 0.05).2.The comparison of AngⅡ(standing and lying position): After treatment,the AngⅡ of patients in the two groups was decreased(P<0.05),and the treatment group was superior to the control group(P < 0.05).3.Comparison of blood lipid-related indicators in patients: After treatment,the control group had no significant improvement in TC,TG,HDL and LDL(P>0.05),while the treatment group had significant improvement in TC,TG,HDL and LDL(P < 0.05).Compared between the two groups after treatment,the improvement of TC,TG,HDL and LDL levels in the treatment group was better than that in the control group(P<0.05).4.The comparison of curative effects on tcm syndromes:(1)Single TCM syndrome: patients in the control group had improvement in vertigo,headache,palpitation and insomnia(P<0.05),but no improvement in soreness of the waist and the knees,dysphoria with feverish sensation in chest,palms and soles,tinnitus and amnesia(P>0.05);In the treatment group,the symptoms such as vertigo,headache,palpitation and insomnia,soreness of the waist and the knees,and dysphoria with feverish sensation in chest,palms and soles were all improved(P<0.05),but there was no significant improvement in tinnitus and amnesia(P>0.05).The efficacy of the treatment group was better than that of the control group in the aspects of vertigo,headache,palpitation and insomnia,soreness of the waist and the knees,and dysphoria with feverish sensation in chest,palms and soles(P<0.05).Neither of the patients in the two groups improved in the aspects of tinnitus and amnesia(P>0.05).(2)The total TCM syndrome scores: After treatment,the total TCM syndrome scores of the two groups were decreased(P<0.05),and the curative effect of the treatment group was superior to that of the control group(P<0.05).(3)The TCM syndrome effective rate: after treatment,the effective rates of the control group and the treatment group were 71.88% and 87.88%,respectively,and the treatment group was superior to the control group(P<0.05).Conclusion:1.Modified Zhengan Xifeng Decoction combined with valsartan could reduce the ambulatory blood pressure of patients with primary hypertension of Yin deficiency and Yang hyperactivity syndrome,effectively improve the blood pressure at night,and effectively improve the blood pressure rhythm of patients with non-dipper hypertension,and promote the change from non-dipper to dipper blood pressure rhythm.2.Modified Zhengan Xifeng Decoction combined with valsartan could reduce the AngⅡ levels in patients with primary hypertension of Yin deficiency and Yang hyperactivity syndrome.3.Modified Zhengan Xifeng Decoction combined with valsartan had the effect of reducing blood lipids in patients with primary hypertension of Yin deficiency and Yang hyperactivity syndrome.4.Modified Zhengan Xifeng Decoction combined with valsartan could improve the clinical symptoms of patients with primary hypertension of Yin deficiency and Yang hyperactivity syndrome,reduce the individual and total scores of the main symptoms of TCM syndrome and most of the concurrent symptoms,and improve the quality of life of patients. |
