Objective: To discuss and analyze the risk factors for metastatic lymph node in the neck region in patients with papillary thyroid microcarcinoma.Methods: The clinical and pathological data of 746 patients with papillary thyroid microcarcinoma who first visited the Department of Oncology in our hospital from September 2020 to September 2021 were collected.The correlation between the factors such as gender,age,tumor size,number of lesions,tumor side,capsular invasion,combined hashimoto’s thyroiditis and cervical lymph node metastasis was analyzed.Result: Univariate analysis showed that a male,younger than 55 years,with multiple lesions,bilateral carcinoma,invasion of the envelope,tumor diameter >5mm,and concurrent hashimoto’s thyroiditis was associated with lymph node metastasis in the central region(all P < 0.05).Another male,with multiple lesions,invasion of the envelope,and tumor diameter >5 mm was associated with lymph node metastasis in the lateral cervical region(all P < 0.05).Multifactorial analysis suggested that independent risk factors for lymph node metastasis in the central region were closely associated with tumor diameter >5 mm,multiple lesions,and bilateral cancer(all P < 0.05).In contrast,independent risk factors for lymph node metastasis in the lateral cervical region were closely associated with tumor diameter >5 mm,invasion of the envelope and multiple lesions(all P < 0.05).Conclusion: When patients with papillary thyroid microcarcinoma are younger than 55 years old,male,with tumor diameter >5 mm,multiple lesions,bilateral cancer,invasion of the capsule,and concurrent hashimoto’s thyroiditis,there is a hi gher risk of lymph node metastasis in the central region.Among them,tumor diameter >5 mm,multiple lesions,and bilateral carcinoma were independent risk factors for the development of metastasis in the lymph nodes of the central region.When patients with papillary thyroid microcarcinoma are male,with tumor diameter >5 mm,multiple lesions,and invasion of the capsule,there is a higher risk of metastasis in the lymph nodes of the lateral cervical region.Among them,tumor diameter >5 mm,invasion of the capsule and multiple lesions were independent risk factors for metastasis in the lymph nodes of the lateral cervical region.
