Clinical Characteristics,treatment And Prognosis Of 168 Patients With Vaginal Intraepithelial Neoplasia

Objective: To study the general clinical characteristics,treatment and prognosisof vaginal intraepithelial neoplasia(VAIN).Methods: The clinical data of 168 patients with histologically confirmed VAINadmitted to the First Affiliated Hospital of Bengbu Medical College from 2019 to February 2022 were retrospectively collected.The sensitivity of different s creening methods to the diagnosis of VAIN and the regression of lesions aftertreatment were compared.Results: The average age of the patients was(49.52±11.63)years old,of whic h 115(68.5%)were over 45 years old,and 57.1% of the patients had menopa use.The overall sensitivity of cytology,HPV,colposcopy and cytology combin ed with HPV screening was 61.5%,92.4%,84.6% and 95.0%,respectively.Th ere was no significant difference in the sensitivity of cytology screening betwe en patients with history of hysterectomy and patients without hysterectomy(P>0.05).The coincidence rate of colposcopic diagnosis and histopathological result s was higher than that of cytological results(P<0.05).The sensitivity of colpos copic diagnosis of VAINII-III was higher than that of Va INI,but the pathologi cal coincidence rate was lower.There was a positive correlation between the g rade of VAIN and the grade of cervical lesions(P<0.05).HPV16 was the highe st infection rate genotype.The highest prevalence of HPV genotypes in patient s with VAINI was HPV16(32.5%),HPV52(25.3%)and HPV58(12.0%).In V AINII-III,HPV16(62.3%)was the highest infection rate type,followed by HP V33(16.4%),HPV52(13.1%)and HPV18(9.8%).Of the 97 patients followedup,83(85.6%)had lesion degradation and lesion regression.Two patients rela psed again after lesion regression,and all improved after retreatment.There we re 5 patients(5.2%)with lesion progression,and 1 patient with VAINIII devel oped into vaginal cancer.Conclusion: The combined detection of cytology and HPV is an important met hod for screening vaginal lesions.We should pay attention to the follow-up ofvaginal lesions in patients with hysterectomy due to HPV-related diseases.Col poscopy plays an important role in locating vaginal lesions.The treatment of VAIN patients depends on the patient ’s basic situation,lesion location and lesi on degree.After active treatment and follow-up,most patients ’ lesions subside d and the prognosis is good.
