| Objective:To explore the relationship between serum apolipoproteins and the characteristics of TCM syndromes in patients with white matter degeneration and cognitive impairment. Methods:A total of 80 patients hospitalized in Xinjiang Uygur Autonomous Region Hospital of Traditional Chinese Medicine and completed head MRI leukodegeneration were enrolled, and MMSE and Mo CA scales were used to divide them into group without cognitive impairment, mild cognitive impairment group and moderate to severe cognitive impairment according to the results.Relevant data were collected from these 80 patients, including gender, age,BMI index, education level, marital status, past history, tobacco and alcohol status, mobile phone use and daily exercise, and relevant laboratory results(apolipoprotein A-1,apolipoprotein B,apolipoprotein B/apolipoprotein A-1,apolipoprotein E,lipoprotein A,and TCM syndrome elements were collected through the "Dementia Syndrome Element Scale".SPSS 21.0 was used for data processing and analysis.Result:In this study,80 patients with white matter degeneration,23 patients with no cognitive impairment,38 patients with mild cognitive impairment, and 19 patients with moderate to severe cognitive impairment were included. The results of the study were as follows: there were significant differences in age, education level, marital status, mobile phone use and LA(a)level between the three groups(P<0.05),and there was a significant difference in myeloid reduction in TCM syndrome elements in the three groups. There were significant differences in language function,MOCA score, visuospatial and executive function, naming, speech, delayed recall, and orientation function, taking myelina reduction as the dependent variable(P<0.05).With no sputum turbidity as the dependent variable, there was a significant difference in Apo A-1 between the two groups(P<0.05).Multivariate analysis of the level of apolipoprotein by taking cognitive impairment as the dependent variable showed that Apo E was an independent risk factor for leukodegeneration with cognitive impairment. The main symptom factors of leukodegeneration with mild cognitive impairment are yin deficiency, hypomyeloidia, impotence and sputum turbidity; The main symptom factors in patients with leukodegeneration with moderate to severe cognitive impairment are qi deficiency, blood deficiency, yin deficiency, and hypomyeloidia. Conclusion: Apo E is an independent risk factor for leukodegeneration with cognitive impairment, and its area under the ROC curve is 0.656(OR=1.073,95% CI: 1.010,1.140),its sensitivity is 70.2%,and its specificity is52.2%;The main symptom factors of leukodegeneration with mild cognitive impairment are yin deficiency, hypomyeloidia, sputum turbidity and impotence; The main symptom factors in patients with leukodegeneration with moderate to severe cognitive impairment are qi deficiency, blood deficiency, yin deficiency, and hypomyeloidia. |
