| ObjectiveNetwork meta-analysis was used to investigate the clinical efficacy ranking of six acupuncture treatments for insomnia.Based on the results of mesh meta-analysis,a randomized controlled clinical study was conducted on the optimal intervention measures and abdominal acupuncture combined with moxibustion Shenque therapy to explore the clinical efficacy of abdominal acupuncture combined with moxibustion Shenque therapy for insomnia.Methods1.The literature research system searched the self-built database of CNKI,Wanfang,VIP and PubMed--the randomized controlled trial(RCTS)on six acupuncture therapies for insomnia published in June 2022,and the two persons independently screened the literature and extracted the data to establish the database. After Cochrane bias evaluation tool was used to evaluate the quality of the literatures that met the nadation criteria,Stata 17.0 software was used to conduct network meta-analysis,and the ranking of each intervention was obtained based on the total clinical response rate.2.In the clinical study,66 insomnia patients who met the inclusion and exclusion criteria were divided into two groups by using a random number table envelope.Thirty-three patients in the acupoint embedding group and the abdominal acupuncture combined with moxibustion Shenque group were treated 12 times, and the clinical efficacy,Pittsburgh Sleep Quality Index Scale(PSQI)score and self-rating Anxiety Scale(SAS)score were recorded before and after treatment.SPSS 26 was used to analyze the research data.Results1.Literature studies(1)Literature screening: 48 studies were finally included, with a total of 13 intervention measures. The intervention measures in the treatment group included18 articles of routine acupuncture,9 articles of abdominal acupuncture,4 articles of electric acupuncture,8 articles of intradermal acupuncture,6 articles of acupoint burying and 3articles of moxibustion.(2)Results of network meta-analysis: abdominal acupuncture, electric acupuncture, intradermal acupuncture, acupoint catgut burial and moxibustion had better efficacy than conventional acupuncture.Network meta-analysis showed that the top three effective rates were acupoint embedding(SUCRA=92.9)> electroacupuncture(SUCRA=81.5)> abdominal acupuncture(SUCRA=78.1).2.Clinical studies(1)Abdominal acupuncture combined with moxibustion Shenque and acupoint catgut embedding can effectively improve the clinical symptoms of insomnia patients, and there were statistically significant differences in PSQI evaluation between the two groups before and after treatment(P < 0.05).(2)Abdominal acupuncture combined with moxibustion Shenque and acupoint catgut embedding had the same effect in improving the clinical symptoms of insomnia, and there was no statistical difference in the total effective rate between the two groups(P > 0.05).There was no statistical difference between the PSQI evaluation groups after treatment(P > 0.05),and there was no statistical difference between the SAS evaluation groups after treatment(P > 0.05).Conclusion1.The results of network Meta-analysis showed that the cumulative probability of total effective rate of six acupuncture and moxibustion therapies for insomnia was ranked as follows: acupoint catgut embedding > electroacupuncture > abdominal acupuncture >moxibustion > intrutaneous acupuncture > conventional acupuncture.2.Compared with the other five acupuncture and moxibustion therapies, acupoint catgut embedding may be the best single intervention for insomnia treatment.3.There was no significant difference between abdominal acupuncture combined with moxibustion Shenque and the improvement of clinical total effective rate, significant efficiency,PSQI total score and SAS score.4.Abdominal acupuncture combined with moxibustion Shenque has significant curative effect in the treatment of insomnia. The treatment process is quiet and comfortable with slight pain, which is worthy of clinical promotion and application. |
