| Objective:The correlation of mechanical tenderness threshold and disease was analyzed by exploring and measuring the sensitive acupoints in patients with primary dysmenorrhea(PD)and comparing with the group without dysmenorrhea.The category of tenderness threshold in each part of the body surface of PD patients is preliminarily proposed.The clinical efficacy of Force-sensitive massage and oral ibuprofen sustained-release capsule in the treatment of PD provides a new idea for the clinical treatment of PD.Methods:1.Test 1: In 80 PD patients who met the criteria,force sensitive acupoint exploration and tenderness threshold were measured.Record on the table and enter into Excel 2010,and count the top five acupoints in frequency.Thirty patients without dysmenorrhea were included to compare the tenderness threshold on the same acupoints to analyze the correlation between the pain threshold and the disease status.And to analyze the range of tenderness threshold in PD patients on the body surface by SPSS 25.0statistics.2.Test 2:A total of 66 PD patients with 4 or more force sensitive acupoints were included in trial 1,which were randomly divided into force sensitive massage group and oral ibuprofen sustained-release capsule group.Both groups are treated on days 1 to 3 of the period,1 menstrual cycle is 1 course,a total of 3 courses of treatment.Before and after treatment,the scale evaluation(COX dysmenorrhea symptom scale and VAS score scale)was performed,and the acupoint tenderness threshold value was measured.The efficacy of patients was also evaluated during the follow-up period.Data analysis was performed by using SPSS 25.0.Results:Test 1 Results:(1)Among the 80 PD patients,69 acupoints were detected,and 58 acupoints were detected,with a total frequency of 401 times,with an average of 5.81 acupoints in each position.According to statistics,the top five acupoints in PD patients are: Taichong,Yinlingquan,Shen Shu,Yin Bao,and Zi Gong,and the top five acupoints with the highest are:foot juyin liver meridian,foot yin spleen meridian,foot sun bladder meridian,foot shaoyin kidney meridian,and foot Yangming stomach meridian.The position coincidence rate of force sensitive acupoints and traditional meridian acupoints accounts for about 69%.(2)Compared with the group without dysmenorrhea,Taichong,Yinlingquan,Shenshu,Yin Bao and Zi Gong all showed low mechanical tenderness threshold,and the two groups were significant(P<0.01).(3)The mechanical tenderness threshold of the body surface of PD patients is as follows: shank: 0.035~0.039Kgf;thigh : 0.036~0.045Kgf;chest and abdomen: 0.033~0.038Kgf;low back 0.040~0.046 Kgf.The average mechanical pain threshold of each site is arranged from low to high in the chest and abdomen,shank,thigh and low back.Test2.Results:(1)Before treatment,the age,duration of disease,degree of disease,VAS score and CMSS score before treatment were compared between the two treatment groups,and the difference was not significant(P>0.05),indicating that the two groups were clinically comparable.(2)After 3 courses,the onset time of CMSS,severity score of CMSS decreased compared with before treatment,and the difference was significant(P<0.01).In comparison between groups,the onset of CMSS dysmenorrhea,CMSS dysmenorrhea severity score and VAS score after 3courses of treatment(P<0.05).In the recent efficacy comparison,the test group and the control group were not statistically significant(P>0.05).(3)During the follow-up period,the duration of dysmenorrhea onset,the severity and VAS score of CMSS decreased compared with before treatment,and the differences were significant(P<0.01).Between groups,the duration of dysmenorrhea onset,CMSS dysmenorrhea severity score and VSS score decreased,and the differences were statistically significant(P<0.01).In terms of long-term efficacy,the test group was better than the control group(P<0.01).(4)According to the statistical analysis,the tenderness threshold value of Force-sensitive acupoints after treatment was significantly higher than that before treatment,in which the difference between Shen Shu,Yin Bao and Zi Gong was significant(P<0.05),and the difference between Taichong and Yinlingquan(P<0.01).The tenderness threshold difference between Taichong and Yinlingquan compared with the group without dysmenorrhea(P<0.05)Conclusion:1.The top five Force-sensitive acupoints with the highest appearance in PD patients are: Taichong,Yinlingquan,Shenshu,Yin Bao,Zi Gong.and the top five acupoints in the meridians are: foot jueyin liver meridian,foot yin spleen meridian,foot sun bladder meridian,foot shaoyin kidney meridian,and foot Yangming stomach meridian.2.Force-sensitive massage and oral ibuprofen sustained-release capsule can effectively treat PD.Both have similar efficacy in the near term,but the long-term efficacy is better than that of western medicine group.3.Mechanical pressure pain thresholds at force-sensitive acupuncture points varied with the condition,and were significantly lower in disease states. |
