| Objective: To apply Gusongbao granule to the rehabilitation of patients with intertrochanteric fracture of femur with deficiency of liver and kidney after operation and observe the curative effect,so as to achieve the clinical effect of short fracture healing time and improve the treatment efficiency in elderly patients.Background: The elderly group is a high incidence group of intertrochanter fracture of the type of deficiency of liver and kidney,and most of the patients are female.The physical tolerance of the elderly patients is worse than that of other age groups,and the recovery cycle after illness is longer,and the mortality and disability rate after fracture have also been high.PFNA minimally invasive surgery for intertrochanter fracture of the elderly is an ideal fixation method,but due to the poor stability of intertrochanter fracture of the femur,it may lead to serious fracture of bone mass and difficulty in reduction.And the follow-up should be strengthened to avoid the failure of internal fixation,if necessary,remedial measures should be taken to carry out refixation,autograft,artificial joint replacement and so on,that is to say,minimally invasive PFNA operation is not once and for all.According to the Chinese Medicine,deficiency of liver and kidney is an important inducement of fracture.The therapeutic schedule should be on the basis of holistic view,and the Chinese and Western Integrative Medicine shows excellent results after verification.In order to consider the treatment efficiency and physical,mental and economic factors of elderly patients,to figure out the quality of the Gusongbao granule after PFNA operation according to the Chinese Medicine and Western Medicine in the cure of liver and kidney yin deficiency fracture.Including whether there are side effects after taking medicine,whether the fracture healing time can be shortened after PFNA minimally invasive surgery,and whether the quality of life of elderly patients can be improved on this basis.Methods: The data of 100 elderly patients with femoral intertrochanteric fracture due to deficiency of liver and kidney were collected from the Department of Orthopaedics of Qingdao Haici Medical Group from October 2019 to October 2022.The selected patients were equally randomized to two groups,there were fifty patients in each groups.Then the treatment,detection and data collection were carried out,and the detailed information eventually is entered into SPSS25.0 software for processing.Patients in the conventional drug therapy group underwent PFNA under combined spinal-epidural anesthesia,and prophylactic antibiotics were given for one day after the operation,supplemented by basic treatment after the operation: detumescence and analgesia,external fixation and functional exercise in the later stage.The methods of drug therapy group are based on the conventional drug therapy group,all drug therapy group patients were given Gusongbao granule with specified measurement from the first day after operation.5 grams each time,three times a day,for 90 consecutive days.Bone mineral density(BMD),bone alkaline phosphatase(BALP),osteocalcin(BGP)and calcitonin(CT)were measured before treatment and on the 14 th,28th,42 nd and 90 th day after operation.During the experiment,the healing time of the fracture site,the swelling index of the thigh and the recovery of hip function were recorded and compared between the drug therapy group and the conventional drug therapy group after minimally invasive surgery.The measured statistics were recorded in time during the experiment and imported into SPSS25.0.Analyze and compared the similarities and differences in detail.It was concluded that Gusongbao Granule had clinical advantages in the treatment of intertrochanteric fracture of liver and kidney yin deficiency type.Results: Before treatment,there was no significant difference in bone mineral density,bone metabolic factor BALP,BGP,CT,the healing time of fracture site and Harris score of hip joint were performed after PFNA minimally invasive surgery for intertrochanter fracture of femurs.And the difference of thigh circumference between two sides(P >0.05).On the basis of the conventional drug therapy group,each patient was given Gusongbao traditional Chinese medicine granule(take it once in the morning,in the middle of the day and in the evening,each time the dose was 5 g).At the end of the study,there were significant differences in statistics before and after the experiment(P<0.05).The experiment results show that the Gusongbao granule obviously shorten the healing period and release the swelling site after PFNA opertion,the rate of treatment increases twenty-two percent overall.The Harris hip scores improved gradually from the first day to ninetieth day,and the drug therapy group patients leg swelling situation released faster than the conventional drug therapy group.Moreover,ninety-four percent drug therapy group patients were disappeared and seventy-six percent of patients had disappeared leg swelling on the fortieth day.The limb movement ability and the quality of life of the patients were improved after operation.In addition,the bone mineral density,bone metabolic factor BALP,BGP and CT were higher than the conventional drug therapy group,and the internal function and physical quality of the patients were enhanced.Conclusion:Gusongbao granule is effective in the recovery of intertrochanter fracture of femur with deficiency of liver and kidney,which can relieve the pain degree of fracture site,relieve the swelling index of operation site,shorten the recovery time of fracture and hip joint function,and improve the effective rate of treatment as a whole.At the same time,after taking Gusongbao granule,the bone mineral density and bone metabolic factors of the elderly patients were increased.In the course of treatment,there were no adverse reactions in the drug therapy group.Taking Gusongbao traditional Chinese medicine granule after operation can improve the physical quality of elderly patients,improve the deficiency of liver and kidney yin,osteoporosis and so on.The quality of life of elderly patients,including after discharge,can be improved,so it should be popularized in clinical practice. |
