| Objective:To compare the pathological prognosis stage with anatomical stage, study the differences between them, and explore the value of pathological prognosis stage in predicting the prognosis of breast cancer patients according to the American Joint Committee on Cancer(AJCC)8th staging system. Methods:A total of 139 primary breast cancer patients in stage I-III with standard surgery as the initial treatment were included in this study.According to the 8th edition of AJCC,the pathological data of patients were sorted out,and the anatomical stage and pathological prognosis stage of each case were determined and compared using the chi-square test.Differences were considered to be significant at p < 0.05.Survival curves were plotted using the Kaplan-Meier method with DFS and OS as endpoints, and the association between the survival and each variable was determined by the log-rank test.Differences were considered to be significant at p < 0.05.Results:The median follow-up was 50 months and the median age was 49 years old.The distribution of anatomical stage and pathological prognosis stage was as follows:anatomical stage: stage IA(36,25.9%),stage IB(0,0.0%),stage IIA(45,32.4%),stage IIB(26,18.7%),stage IIIA(23,16.5%),stage IIIB(0,0.0%),stage IIIC(9,6.5%).Pathological prognosis stage: stage IA(59,42.4%),stage IB(36,25.9%),stage IIA(17,12.2%),stage IIB(6,4.3%),stage IIIA(14,10.1%),stage IIIB(5,3.6%),stage IIIC(2,1.4%).In 80(57.6%)patients the stage change. The most significant change was downstaging. It’s different between anatomic stage and pathological prognosis stage. And both of them are significantly related to prognosis.Compared to patients with unchanged stage, patients with 1-level and 2-level downstaging had better 3-year DFS rates.Compared with anatomical stage, the pathological prognosis stage provides better prognosis information. The Kaplan-Meier curves for the disease-free survival(DFS)and overall survival(OS)were better stratified by the pathological prognosis stage. Conclusion:It’s different between the anatomical stage and pathological prognosis stage of breast cancer. Both of them can predict the prognosis of breast cancer patients to a certain extent. The pathological prognosis stage gives better and more accurate prognosis information than anatomical stage, which has certain reference significance for clinical practice. |
