Objective:To evaluate the clinical significance of red cell distribution width(RDW)in the occurrence,exacerbation,and poor prognosis of chronic obstructive pulmonary disease(COPD).Methods:By searching Pub Med,Embase,Cochrane Library,CNKI,Wanfang Database,VIP database,China Biology Medicine disc(CBM)and other databases by computer,the relevant studies on RDW and COPD were comprehensively collected.The retrieval time was from the establishment of the database to October 2022.The quality of the included literatures was evaluated according to NOS rating table.Meta-analysis was performed using Rev Man 5.4 software at RDW level between healthy control group and stable COPD(S-COPD)group,S-COPD group and exacerbation of chronic obstructive pulmonary disease(ECOPD)group,survival group and death group.Results:After searching the database,a total of 864 literatures were obtained.After repeated screening,20 studies were finally included for meta-analysis.The results showed that:(1)In 8 studies involving 1208 patients with S-COPD and 1034 healthy controls,RDW level in S-COPD group was higher than that in healthy control group(WMD=0.94,95%CI= 0.30-1.57,P < 0.05),and the difference was statistically significant.(2)In 8 studies involving 1471 patients with ECOPD and 901 patients with S-COPD,RDW level in ECOPD group was higher than that in S-COPD group(WMD=1.78,95%CI=0.86 ~ 2.70,P < 0.05),and the difference was statistically significant.(3)In 8 studies involving 308 COPD patients in the death group and2370 COPD patients in the survival group,RDW in the death group was higher than that in the survival group(WMD=1.35,95%CI= 1.09-1.61,P < 0.05),and the difference was statistically significant.Conclusion:The results of this meta-analysis showed that the red cell distribution width level in stable COPD patients was higher than that in healthy control group,that in aggravated COPD patients was higher than that in stable COPD patients,and that in death COPD patients was higher than that in survival group.This suggests that red cell distribution width has important clinical significance for COPD exacerbation and prognosis assessment,which should be paid attention to by clinicians.
