Objective:By comparing the ratio of mononuclear cell count to high density lipoprotein cholesterol in gestational diabetes and normal glucose tolerance pregnant women in second-trimester differences,To investigate the relationship between the ratio MHR with GDM and the value of prediction for GDM,as well as analysis the influence factors of MHR ratio.Methods:A retrospective analysis was made on pregnant women who delivered singletons in Hospital from January 2020 to January 2022.in accordance with the diagnostic standards and in corporations and exclusion standards,We were selected 519 pregnant women.including 289 pregnant women with GDM and 230 pregnant women with normal glucose tolerance test,collecting blood analysis,biochemical indicators,and serum 25-(OH)-D levels at 24-28 weeks of pregnancy.calculate clinical data,the mean ±standard deviation(± S)was used to describe the normal distribution or approximately normal distribution,and the skewed distribution is described by the median(upper quartile,lower quartile).The data satisfying normal distribution and uniform variance are tested by independent sample t-test,while the data with skewed distribution and uneven variance are compared by Mann-Whitney U test.The counting data are described by frequency or percentage,and the chi-square test is compared between groups.Spearman was used to test the correlation of various factors.The significant indicators were analyzed by binary Logistic regression analysis to verify the relationship between each index and GDM.The receiver working characteristic curve(AUC)was calculated to analyze the predictive value of MHR ratio for GDM.Results:1.In the GDM group,the age,pre-pregnancy weight and pre-pregnancy body mass index of pregnant women were higher than those in the NGT group,with(P<0.05).2.Pregnant women in the GDM group were higher in LC,SUA,TG,FPG,1h PG,2h PG,Hb A1 c,MHR,TG/HDL-C than in the NGT group.HDL-C and 25-(OH)-D in the GDM group were lower than those in the NGT group,with(P<0.05).3.Spearman correlation analysis showed that the ratio of MHR in the second trimester was positively correlated with TG/HDL-C,TG,FPG,1h PG and Hb A1 c,and significantly negatively correlated with 25-(OH)-D,(P<0.05).4.The results of multivariate logistic analysis showed that the MHR ratio and TG/HDL-C were independent risk factors for GDM,with(P<0.05).5.ROC curve results show that the AUC of MHR ratio for predicting GDM is 0.642,(P<0.05).The AUC predicted by TG/HDL-C for GDM is0.667,(P<0.05).The AUC of the two indicators for predicting GDM is0.688,(P<0.05).6.The incidence of premature rupture of membranes,hyper bilirubinemia and neonatal transfer to pediatrics in GDM group were higher than those in NGT group,(P<0.05).the blood glucose at birth in newborns was significantly lower than that in NGT group,(P<0.05).Conclusions:1.The ratio of MHR women with GDM during the second trimester was significantly higher than that of women with NGT,and an elevated the ratio of MHR in the second trimester was an independent risk factor for the development of GDM.2.The MHR ratio was positively correlated with the TG/HDL-C ratio of the IR evaluation index,and the higher the MHR ratio,the stronger the IR.MHR ratio was negatively correlated with 25-(OH)-D,and the increase of MHR ratio may be related to low Vit D levels.3.The ratio of MHR has predictive value for GDM.
