Objective: To observe the clinical efficacy of Jiangtang Huoluo Decoction in the treatment of diabetic peripheral neuropathy,evaluate its safety,and preliminatively explore the mechanism of action of Jiangtang Huoluo Decoction in the treatment of diabetic peripheral neuropathy.In order to provide scientific basis for TCM treatment of diabetic peripheral neuropathy.Methods: In this study,72 patients with diabetic peripheral neuropathy who met the requirements of the protocol were included.The patients were divided into control group and treatment group by random number table method,with 36 patients in each group.I In terms of treatment,both groups were given appropriate western medicine basic treatment such as hypoglycemia,hypertension and lipid regulation,supplemented by routine health education,reasonable distribution of diet and appropriate exercise.On the basis of this,the treatment group was given Jiangtang Huoluo decoction(two times a day),the control group was given mecobalamin tablets(0.5mg three times a day),3 weeks as a course of treatment,continuous observation of 2 courses.TCM syndrome score,biochemical indexes,blood glucose,TCSS score and nerve conduction velocity were recorded before treatment and after two courses of treatment.To observe the improvement of symptoms and TCM syndromes of diabetic peripheral neuropathy at the end of treatment,and to observe the occurrence of hypoglycemia and other adverse reactions throughout the treatment.To evaluate the safety and efficacy of Jiangtang Huoluo decoction in treating diabetic peripheral neuropathy with qi deficiency and blood stasis.Results:1.Blood glucose levels: After treatment,FBG,2h BG and Hb A1 c in both groups decreased compared with before treatment,but compared with the control group,blood glucose in the treatment group decreased more significantly(P<0.05).2.Homocysteine: After 6 weeks of treatment,homocysteine decreased in both the treatment group and the control group(P<0.05),but the decreased level was higher in the treatment group,indicating a significant comparison between groups(P<0.05).3.Electromyography: After treatment,nerve sensory and motor conduction velocity were improved in both groups(P<0.05),but the improvement of nerve conduction velocity in treatment group was more obvious than that in control group(P<0.05).4.Efficacy of peripheral neuropathy: the total effective rate of the treatment group was87.5%,while that of the control group was only 68.6%;TCSS score: TCSS scores in both groups decreased after 6 weeks of treatment(P<0.05).However,the TCSS score of the control group was significantly lower than that of the treatment group(P<0.05).5.TCM syndrome effect: The total effective rate was 90.6% in the treatment group and71.4% in the control group.The TCM syndrome score of both groups decreased after treatment(P<0.05),but the decrease level of the treatment group was more significant than the control group(P<0.05),and the improvement effect of limb weakness,limb numbness,limb cooling,shortness of breath and weakness was more prominent(P<0.05).6.Safety analysis: During the observation period of this study,there were no adverse reactions in either group.Conclusion: Jiangtang Huoluo decoction has beneficial effects of promoting qi and blood circulation,clearing collaterals and relieving pain,improving TCM clinical symptoms,increasing nerve conduction velocity and reducing TCSS score,and alleviating diabetic peripheral neuropathy by alleviating oxidative stress and reducing nerve cytotoxicity.
