| Objective:To observe the clinical efficacy of Shenqi Gualou Xiebai Banxia Decoction and Sanren Decoction in the treatment of patients with coronary heart disease,angina pectoris combined with chronic heart failure(Qi Xu Tan Zu,Shi Re Yun Jie).Further to verify the effectiveness and safety of Shenqi Gualou Xiebai Banxia Decoction and Sanren Decotion during the treatment and summarize the tutor’s clinical experience in treating the disease and explore its reasons and principles.Materials and Methods:Collect 119 medical records of patients with coronary heart disease and angina pectoris combined with chronic heart failure(Qi Xu Tan Zu,Shi Re Yun Jie)in the Department of Cardiology and outpatients clinical of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2016 to August 2019.These records are divided into two groups:chinese medicine treatment group(basic western medicine treatment plus Shenqi Gualou Xiebai Banxia Decoction and Sanren Decoction plus or minus)and basic treatment group(basic western medicine treatment)according to the differences among therapeutic regimens.The course of treatment lasted for 4 weeks.The clinical data of TCM syndromes,electrocardiogram,cardiac color Doppler ultrasound,and cardiac function of the two groups of patients before and after the intervention were collected and compiled into Excel tables.The SPSS 26.0 software was used to statistically analyze the improvement of the patients in the TCM syndrome scores,ECG,NT-pro BNP level,cardiac color Doppler ultrasound,cardiac function and therapeutic effects before and after the treatment.Results:1.TCM syndromes: After treatment,the patient’s chest pain,chest tightness,shortness of breath,fatigue,edema of facial limbs,abdominal distension,dizziness,palpitations,cough,asthma,loose stools,oliguria,tongue quality,tongue coating,pulse condition,etc.were all improved with statistical significance(P<0.05),and the chinese medicine treatment group was significantly better than the basic treatment group(P<0.05).TCM syndrome score: before treatment,the TCM syndrome score of the chinese medicine treatment group was(28.25±2.222),and that of the basic treatment group was(28.77±0.247);after treatment,the TCM syndrome score of the chinese medicine treatment group was(5.86±3.512),the TCM syndrome score of the basic treatment group was(14.74±4.008),and the curative effect of the chinese medicine treatment group was significantly better than that of the basic treatment group(P=0.000 <0.01).Efficacy of TCM syndromes: 50 cases in the chinese medicine treatment group were markedly effective,accounting for 87.72%,5 cases were effective,accounting for 8.77%,and the total effective rate was 96.49%;3 cases in the basic treatment group were markedly effective,accounting for 4.84%,51 cases were effective,accounting for82.26%,and the total effective rate was 87.10%,and the curative effect of the chinese medicine treatment group was better than that of the basic treatment group(P<0.05).Efficacy o2.ECG improvement: 22 cases were markedly effective in the chinese medicine treatment group,accounting for 38.60%,27 cases were effective,accounting for 47.36%,and the total effective rate was 85.96%;12 cases in the basic treatment group were markedly effective,accounting for 19.35%,and 33 cases were effective,accounting for 53.23%.The effective rate was 72.58%,and the curative effect of the chinese medicine treatment group was better than that of the basic treatment group(P <0.05).3.NT-proBNP level: the average value of NT-proBNP in the chinese medicine treatment group before treatment was(3799.33±907.556)pg/ml,the average value of NT-pro BNP in the basic treatment group was(3711.53±951.226)pg/ml,after treatment the average NT-pro BNP in the chinese medicine treatment group The value was(1269.05±687.887)pg/ml,the average value of NT-pro BNP in the basic treatment group was(1827.84±1019.384)pg/ml,and the chinese medicine treatment group had better efficacy than the basic treatment group(P <0.05).4.Left ventricular ejection fraction: The average left ventricular ejection fraction(EF)of the chinese medicine treatment group before treatment was 46.68%±2.84%,and the average of the basic treatment group was 46.82%±3.08%;the left ventricle of the chinese medicine treatment group after treatment The average ejection fraction was 50.72±2.305%,and the average left ventricular ejection fraction of the basic treatment group was 48.10%±2.245%,and the chinese medicine treatment group was better than the basic treatment group(P<0.05).5.In terms of cardiac function: 33 cases were markedly effective in the chinese medicine treatment group,accounting for 57.89%,17 cases were effective,accounting for 29.83%,and the total effective rate was 87.72%;21 cases in the basic treatment group were markedly effective,accounting for 33.87%,and 24 cases were effective,accounting for 38.71%.The effective rate is 72.58%.And the curative effect of the chinese medicine treatment group was significantly better than that of the basic treatment group(P<0.05).Conclusion:1.The pathogenesis of this disease is rooted in the deficiency of qi and yang,and manifested as turbid phlegm,dampness and heat.The disease location is in the heart and closely related to the lungs,spleen,kidneys and sanjiao.Therefore,the treatment should focus on the viscera and Sanjiao at the same time and strengthen vital qi to eliminate pathogenic factor on the basis of overall concept so as to diagnose pathogenesis and address both the symptoms and root cause.2."Yiqi Huatan,Qingre and Lishi" method,the prescription uses Shenqi Gualou Xiebai Banxia Decoction and Sanren Decoction to treat coronary heart disease angina pectoris combined with chronic heart failure(Qi deficiency,phlegm obstruction,damp-heat accumulation syndrome)can effectively improve myocardial blood supply,reduce heart failure indicators and improve cardiac function.Further more patients’ quality of life can also be improved.It has been confirmed that the clinical efficacy is obvious and guaranteed. |
