Purpose:To observe the clinical effect of giant thorn in the treatment of spastic paralysis of upper limbs after stroke,and to provide a more effective means in clinical treatment.Material and method:Sixty subjects who met the current research criteria were randomly divided into observation group(30 cases of giant acupuncture group)and control group(30 cases of antagonistic muscle acupuncture group).Patients in both groups received basic neurology treatment and upper limb rehabilitation function training.The observation group was treated with giant thorn method,and the control group was treated with antagonistic muscle acupuncture.The two groups were needled once a day for 30 minutes each time for 6 consecutive days,and rested and evaluated on the 7th day.The modified Barthel index(MBI)(upper extremity),the modified Ashworth Spasm Scale(MAS)and the simplified Fugl-Meyer Motor Function Scale(FMA)(upper extremity)were applied to the subjects’ daily living ability,spasm grade and motor function before treatment,on the 7th day and the 14 th day after treatment Objective assessment was performed to observe the difference in clinical efficacy between the two groups.Results:1.There was no case of shedding in the two groups in this study,and the baseline data and scale evaluation before treatment were compared,which were comparable after statistical analysis.2.MBI,MAS and FMA scores of two groups were significantly improved before treatment and after treatment,and the differences were statistically significant,(P<0.05).3.Modified Barthel index evaluation: On day 7,there was no significant change in the scores of the two groups,which did not immediately improve the ability of daily living of the upper limbs.At day 14,there was no significant difference in scores between the two groups,P>0.05,and the scores in the two groups were significantly changed compared with those before treatment,and the difference was statistically significant(P<0.05).4.Modified Ashworth Spasticity Scale Assessment: On the 7th day,when acupuncture was performed in the giant needling method,the affected limb was passively moved,and the degree of spasticity was significantly lower than that before treatment(P<0.05),indicating that the giant needling method group could reduce the increase in muscle mass.The degree of tension was better than that of the control group,and there was an immediate effect;on the fourteenth day,the efficacy scores between the two groups(P>0.05),The difference was not statistically significant,but there was significant difference between the two groups compared with the untreated group(P<0.05).5.Simplified FUGL-Meyer Scale of Motor Function assessment: At day 7,there was no significant difference in the improvement of motor capacity in the affected limb between the two groups(P>0.05),had no immediate effect on the improvement of motor ability of affected side;At day 14,there was no significant difference in scores between the two groups(P>0.05),and there was significant difference in scores between the two groups compared with those without treatment(P<0.05).Conclusion:1.Giant needling method and antagonistic muscle acupuncture can effectively reduce the degree of upper limb spasm after stroke,improve the movement ability of the affected limb,and improve daily living ability and prognosis;2.On the 7th day of treatment,from the short-term evaluation effect,the giant needling method has obvious advantages in reducing spasm during passive movement of the affected limb,and has an immediate effect;3.After the 14-day course of treatment,from the long-term evaluation results,the giant needling method and acupuncture antagonistic muscle have significant curative effects in improving the spasticity of the affected limb,exercise ability and daily living ability after stroke,but the curative effect between the two groups No significant difference.
