Clinical Observation On Modified Huanglian Jiedu Decoction In The Treatment Of Sepsis-related Acute Kidney Injury With Toxic Heat Syndrom

Objective:To observe the clinical effect of modified Huanglian Jiedu Decoction on sepsis-associated acute kidney injury.Methods:Patients who met the diagnostic criteria of sepsis and acute kidney injury and belonged to the syndrome of the exuberance of internal heat and toxin were selected and divided into treatment group and control group by randomized controlled study classification.There are 25 people in the actual treatment group and 22 people in the control group.The control group was treated with routine treatment,and the treatment group was given oral or nasal feeding with modified Huanglian Jiedu decoction on the basis of control group.Observing urine volume,renal function,inflammation index,symptom score and SOFA score before and after treatment to evaluate its effectiveness and safety.Results:1.TCM syndrome score and SOFA score of patients in 2 groups decreased after treatment(P≤0.05).TCM syndrome score and SOFA score decreased more significantly in the treatment group(P≤0.05).The total effective rate of the treatment group was 96%,and that of the control group was 95.45%.There was no statistical difference in the efficacy of TCM syndrome between the two groups(p>0.05).2.GFR,24h urine volume,serum creatinine and other renal function indexes in both groups were improved after treatment(P≤0.05).GFR,24-hour urine volume,serum creatinine and other renal function indexes improved more significantly in the treatment group(P≤0.05).3.WBC,CRP and PCT in 2 groups decreased after treatment(P≤0.05).The decrease of WBC,CRP and PCT in the treatment group was more significantly(P≤0.05).4.There were no significant differences in lactic acid value and IL-6 level between the two groups before and after treatment(p>0.05).Conclusion:The results of this study show that the treatment of Western medicine combined with modified Huanglian Jiedu decoction can reduce the level of inflammatory factors,the inflammatory response and SOFA score,improve clinical symptoms and renal function of sepsis-associated acute kidney injury patients with the syndrome of the exuberance of internal heat and toxin more quickly.
