Objective: The purpose of this study was to evaluate the effect of Dachaihu decoction combined with ursodeoxycholic acid on bile metabolism after LCBDE of hepatobiliary damp-heat choledocholithiasis by observing and comparing the related indexes between the experimental group and the control group.Methods: In this study,patients with choledocholithiasis who underwent LCBDE were selected from January 2022 to December 2022 in the Department of Hepatobiliary Pancreatic and splenic surgery of Hunan Provincial traditional Chinese Medicine Hospital,and they were classified as hepatobiliary damp-heat syndrome by TCM syndrome differentiation analysis.The subjects were randomly divided into experimental group(n =25)and control group(n = 25).The control group was treated with basic treatment plus ursodeoxycholic acid,and the experimental group was treated with Chaihu decoction on the basis of the control group.The clinical efficacy and safety of the two groups were observed after treatment.Results: 1.In this study,there was no significant difference in sex,age,common bile duct diameter and cholecystectomy history between the experimental group and the control group(P > 0.05).2.The levels of serum glutamic pyruvic transaminase and glutamic oxaloacetic transaminase in the experimental group were significantly lower than those in the control group at 4 and 8 days after operation(P < 0.05).3.The levels of total bilirubin and indirect bilirubin in serum and bile in the experimental group were significantly lower than those in the control group at 4 and 8 days after operation(P < 0.05).There was no significant difference in the level of direct bilirubin in serum and bile between the two groups(P > 0.05).4.There was no significant difference in the levels of total bile acid and total cholesterol in serum and bile between the two groups at 4 and 8 days after operation(P > 0.05).5.There were significant differences in the levels of total cholesterol and total bile acid in serum and bile before and after treatment of Dachaihu decoction combined with ursodeoxycholic acid in the experimental group(4 days and 8 days after operation)(P < 0.05).In the control group,there were significant differences in the levels of total cholesterol and total bile acid in serum and bile before and after ursodeoxycholic acid treatment alone(4 days and 8 days after operation)(P< 0.05).Conclusion: 1.Dachaihu decoction can effectively reduce the levels of serum glutamic pyruvic transaminase and glutamic oxaloacetic transaminase in patients after LCBDE,and has the effect of protecting liver.Dachaihu decoction can effectively reduce the levels of total bilirubin and indirect bilirubin in patients after LCBDE,and inhibit the formation of bilirubin pigment stones.2.Ursodeoxycholic acid can increase the level of bile acid,reduce the level of cholesterol and inhibit the formation of cholesterol stones after LCBDE;combined with Dachaihu decoction can not enhance the effect of ursodeoxycholic acid on bile acid and cholesterol metabolism.
