Purpose : Based on the theory of heart and liver and gallbladder,the distribution characteristics of TCM symptoms in patients with coronary heart disease were observed,and the relevant physical and chemical examination results were analyzed to explore the correlation between heart and liver and gallbladder,so as to provide clinical basis for TCM theory.Material and method:A total of 150 inpatients in the Department of Cardiology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected.The data were collected and sorted out.The distribution of TCM symptoms in patients was collected by inquiry.The distribution trend of TCM symptoms in patients with coronary heart disease was analyzed by frequency statistics.The results of coronary angiography were calculated by Gensini score.The results of hepatobiliary and splenic color Doppler ultrasound and liver function were sorted out and entered into Excel table.SPSS 26.0 statistical software was used to analyze the data.Results:1.Distribution of TCM symptoms of coronary heart disease(1)The frequency of TCM symptoms of coronary heart disease from high to low is spleen 92(61%),liver and gallbladder 83(55%),kidney 60(40%),lung 43(29%).(2)In the distribution of TCM symptoms,the frequency of symptoms with two disease locations from high to low is heart,liver and gallbladder(19),heart and spleen(16),heart and lung(8),heart and kidney(4).The frequency of symptoms combined with three disease locations from high to low is heart,liver,gallbladder and spleen(28),heart,spleen and kidney(16),heart,liver,gallbladder and kidney(13),heart,spleen and lung(12),heart,kidney and lung(7),heart,liver,gallbladder and lung(5).The frequency of symptoms combined with four disease locations from high to low is heart,liver,gallbladder,spleen and kidney(11),heart,spleen,kidney and lung(4),heart,liver,gallbladder,spleen and lung(2),heart,liver,gallbladder,kidney and lung(2).The frequency of symptoms combined with five disease locations was heart,liver,gallbladder,spleen,kidney and lung(3).2.TCM syndromes of coronary heart disease and age and sex distribution(1)Among the 150 patients with coronary heart disease selected,the proportion of TCM syndromes was 51(34%);qi deficiency and blood stasis type 36 people(24%);qi and Yin deficiency type 33 people(22%);qi stagnation and blood stasis type 30 people(20%).(2)The gender of patients with coronary heart disease was statistically significant in different TCM syndromes(P<0.05),and the age of patients with coronary heart disease was statistically significant in different TCM syndromes(P<0.05).3.Correlation between symptom distribution and ageThe patients aged 40-59 years old had the most symptoms of heart,liver,gallbladder and spleen,accounting for 31%.The number of patients with heart and spleen and heart,spleen and kidney symptoms in 60-80 years old was the highest,accounting for 14%.4.Correlation between TCM syndromes of coronary heart disease and location of five zang organs(1)Qi deficiency and blood stasis type of TCM symptoms of disease frequency from high to low in order for heart and lung(7),heart spleen and kidney(6),heart liver gallbladder kidney(5),heart liver gallbladder lung(4),heart kidney lung(4),heart liver gallbladder spleen kidney(3),heart spleen(2),heart kidney(2),heart liver gallbladder kidney lung(1),heart spleen lung(1),heart spleen kidney lung(1).(2)The frequency of TCM symptoms of phlegm and blood stasis type from high to low is heart,liver,gallbladder and spleen(19),heart and spleen(9),heart,spleen and lung(6),heart,liver,gallbladder,spleen and kidney(5),heart,spleen and kidney(4),heart,liver and gallbladder(2),heart,spleen,kidney and lung(2),heart,liver,gallbladder,spleen,lung(1),heart,liver,gallbladder,kidney and lung(1).(3)The frequency of TCM symptom location of Qi and Yin deficiency type from high to low is heart,spleen and kidney(6),heart and spleen(5),heart,spleen and lung(5),heart,liver,gallbladder and kidney(4),heart,kidney and lung(3),heart and kidney(2),heart,liver,gallbladder,spleen and kidney(2),heart and lung(1),heart,liver,gallbladder and lung(1),heart,liver,gallbladder and spleen(1),heart,liver,gallbladder,spleen and lung(1),heart,liver,gallbladder,spleen,kidney and lung(1),heart,spleen,kidney and lung(1).(4)The frequency of TCM symptoms of qi stagnation and blood stasis from high to low is heart,liver and gallbladder(17),heart,liver,gallbladder and spleen(8),heart,liver,gallbladder and kidney(3),heart,liver,gallbladder,spleen and kidney(1),heart,liver,gallbladder,spleen,kidney and lung(1).5.Correlation analysis between TCM syndromes of coronary heart disease and liver functionThere were statistically significant differences in direct bilirubin,total bile acid,blood lipid and alanine aminotransferase among TCM syndromes of coronary heart disease(P<0.05).There was no significant difference in total bilirubin and aspartate aminotransferase between TCM syndromes of coronary heart disease(P>0.05).6.Analysis of color Doppler ultrasound and liver function results(1)The results of hepatobiliary and spleen color Doppler ultrasound showed that from high to low were gallstones 54(36%),mild fatty liver 50(33%),moderate fatty liver 23(15%),gallbladder polyps 10(7%),liver cysts 3(2%),cholecystectomy 2(1%).(2)The direct bilirubin and high density lipoprotein in group A1 were significantly higher than those in group A2,and the difference was statistically significant(P<0.05).The total bilirubin,total bile acid,triglyceride,alanine aminotransferase,aspartate aminotransferase and Gensini score in group A2 were significantly higher than those in group A1(P<0.05).There was no significant difference in total cholesterol and low density lipoprotein(P>0.05).7.Correlation analysis of coronary artery stenosis and liver functionThe level of direct bilirubin was negatively correlated with the degree of coronary artery stenosis(P < 0.05).Total bile acid,triglyceride,total cholesterol and alanine aminotransferase were positively correlated with the degree of coronary artery stenosis(P<0.05).There was no correlation between total bilirubin level and coronary artery stenosis(P>0.05).Conclusion:1.Patients with coronary heart disease are prone to symptoms related to liver and gallbladder,which can provide clinical basis for the theory of traditional Chinese medicine and provide new ideas for clinical diagnosis and treatment of diseases.2.With the increase of age,the related organs will appear abnormal.3.In different TCM syndromes of coronary heart disease,total bile acid,triglyceride,total cholesterol,low density lipoprotein and alanine aminotransferase were significantly higher than other syndromes.4.Direct bilirubin,high density lipoprotein,total bilirubin,total bile acid,triglyceride,alanine aminotransferase,aspartate aminotransferase,Gensini score and coronary heart disease and hepatobiliary have potential links.5.The level of direct bilirubin was negatively correlated with the degree of coronary artery stenosis.Total bile acid,triglyceride,total cholesterol and alanine aminotransferase were positively correlated with the degree of coronary artery stenosis.Coronary heart disease has a certain correlation with hepatobiliary function.
