| ObjectiveThe paper analyze the existing problem in the prevention and control work of HIV/AIDS in Henan from three aspects:the manager,the provider and the consumer. To carry out systematic study the strategies of comprehensive prevention and control of HIV/AIDS.To describe and analyze the current operating situation of the model of comprehensive prevention and control of HIV/AIDS,identify the key factors that impact the model development,learn from experience of HIV/AIDS prevention and control at home and abroadand,and developing strategy and raise the sustainable development strategy on public management theory.MethodsResearch objects:the four counties with high incidence of AIDS in Henan Province-Shenqiu, Shangcai, Weishi, Queshan-are the research sites. Then,2 to 3 towns were selected from each county and 1 to 2 villages were selected from each town as the field investigation sites. In every village,25 to 35 families with HIV/AIDS members were randomly selected as the intervention group and similar families with no HIV/AIDS members were also selected as the control group. By questionnaire research, the data would be comparative analyzed with that of 2004. At the end,1200 families in intervention group and 1200 families in control group were involved in this research which included 1129 HIV/AIDS patients and 1155 no HIV/AIDS people. Regarding to the health institutions,4 CDC,8 designated medical institutions (4 county hospitals and 4 Maternal and Child Health Hospitals) in counties and 10 township hospitals as well as 33 village clinics were in this research. The investigation content includes infrastructure development, AIDS prevention and control work, the quality and training of staff engaged in AIDS prevention and treatment. Methods:Quantitative and qualitative methods were applied to carry out supplementary surveys. Qualitative survey included in—depth interview, focus-group discussion etc. Quantitative survey included investigation for mechanism and institution of the AIDS comprehensive prevention and control, the comprehensive quality and training of personnel, the health services utilization of HIV/AIDS, family disease financial burdens, and AIDS knowledge awareness, and AIDS patients living quality.The data analysis methods including descriptive research, multi factor analysis,factor analsysis, SWOT analysis and scenario analysis etc.Results1.The model of HIV/AIDS prevention and contro in Henan led by government which including three supporting systems:Control and Prevention system,medical care system and assistance system. After discussion of topics group find out the ten following problems existing:discrimination of AIDS patients bring potential social problem; fund for AIDS prevention and control; the legalization of the normalization of AIDS patients and management; execution of AIDS policy should strengthen; infrastructures of medical institutions are deficiency; remuneration of personnel; the lack of technology training of medical personnel; improve protection consciousness of HIV/AIDS prevention and control personnel; the fairness of the affected population of AIDS etc.2. The funding of AIDS prevention and control in Henan province including government investment and international support, the government investment funds from provincial, city and county level, the funds investment and spending are basically flat in 2007-2009.There are existing the following problems:the time of funds allocated is too long; the system of budgeting and management should be adjust etc.3.The work of prevention and control including voluntary consultation detection, maternal and infant blocking, target population management and regular inspection, health education; the current AIDS patients management way is not suitable for floating population; the work of maternal and infant block still need to strengthen.4. the medical institution of township and village level is the key construction project of the medical treatment service network, and the main existing problems are lack of equipment,the capacity for treatment cannot meet demand.5. Social relief mechanism for AIDS patients is relatively perfect including the education and production assistance, but the mechanism sustainability remain to be discussed.6. Compared with the average of the whole nation, the human resource of AIDS comprehensive prevention and control is justified. Lack of the senior professionals in the team of grass root. Training is comparatively backward. The subject of training lack of the occupational exposures and mental health for medical staff; The workload of prevention and control is larger, the income level for personnel is low, and lack of occupational protection and security measures after occupational exposure.7. The degree of health services utilization of HIV/AIDS is increasing, medical institutions focus on rural two level, the burden of basic-level medical institutions is heavier, should strengthen the ability of basic medical and health institutions.8. The awareness of HIV/AIDS of AIDS knowledge is better than the general population, but is not comprehensive. Many factors analysis showed that sex, age, cultural awareness level are the main influence factors. The knowledge, attitude and behaviors can not unified will become the important issue effective in HIV/AIDS prevention and control work.9. The score of quality of life for HIV/AIDS,except for the spiritual support/personal beliefs field higher than the Chinese norm, other areas and total score points are lower than Chinese norms.Dicussion and suggestion1.According to the public management theory, take example by the advanced experience of HIV/AIDS prevention and control in domestic and foreign, and proposes to construct the social support system suitable for HIV/AIDS patients in henan province.The system shall bear the following tasks:the first, provide health care services and professional psychological guidance for HIV/AIDS; the second,it guide AIDS patients and their families to deveolp self-reliance in order to obtain the economic sources; the third, spread the related knowledge of HIV/AIDS prevention and control,to increase HIV/AIDS prevention and control of knowledge awareness and reduce social discrimination.2. Strategies and recommendations of AIDS prevention and control development in Henan Province(1) Improving the methods and content of health education and promotion and creating the social environment conducive to the AIDS control.(2) Strengthen the service capacity of the medical and health institutions and improve the healthcare providers' job security and income.(3) Give full play to China's charity class fund to protect AIDS prevention and rescue work ongoing.(4) Gradually improve the national referral system and to achieve unified management of the floating AIDS patients.(5) Build AIDS prevention and community system as well as family support system.
