| This research is to find out the relationship between kidney-yang-deficiency syndrome and the transcription factors T-bet/GATA-3expression, and to investigate the effect of kidney-yang warming therapy on the T-bet/GATA-3expression and on tissue remodeling.Research method, result1. Clinical research1.1Research method:Totally91Kidney-yang-deficiency patients, including treatment group composed of41cases, control group with50cases and20healthy human were recruited in this study. The treatment group was given Jinkui Shenqi pill and control group was given Loratadine pills.The expression of T-bet and GATA-3in the peripheral blood mononuclear cells were detected by using semi-quantitative PCR and flow cytometry analysis system.1.2Result:After treated with Kidney-yang Warming Therapy, expression of INF-γ,T-bet in allergic rhinitis patients was greatly improved (P<0.05) and expression of IL-4,GATA-3was decreased(P<0.05). Expression of INF-γ,T-bet IL-4and GATA-3in control group did not change (P>0.05)2. Experiment research2.1Material with method Kidney-yang-deficiency male guinea pigs models were firstly induced by adenine and then were sensitizated by inhalation of ovalbumin to establish allergic rhinitis models. The treatment group was given Jinkui Shenqi pill and control group was given Loratadine pills..After treatment,the changes of behavior, secretion of nasal cavity and nasal mucosa were observed.The levels of INF-γ,T-bet, IL-4and GATA-3were detected in the peripheral blood mononuclear cells were by using semi-quantitative PCR and flow cytometry analysis system. At24h after the last intranasal challenge, the guinea pigs were killed and the heads of the animals were removed and fixed in10%neutral buffered formalin for24hours, then decalcified in5%trichloroacetic acid for10days. The tissue blocks were embedded in paraffin. The paraffin sections3m thick were stained with hematoxylin and eosin (HE), alcian blue (pH,2.6)-periodic acid-Schiff (AB-PAS), and Masson's Trichrome (MT). The infiltrating eosinophils in nasal mucosa were examined, AB-PAS-positive cells in the surface epithelium in nasal septal mucosa were counted. The percentage area of MT stained extracellular matrix in septal mucosa and conchae and damage of epithelium were determined by an image analyzer2.2Result2.2.1The expression of INF-γ, T-bet, IL-4and GATA-3After treated with Kidney-yang Warming Therapy, expression of INF-γ,T-bet in allergic rhinitis patients was greatly improved (P<0.05) and expression of IL-4,GATA-3was decreased (P<0.05). Expression of INF-γ, T-bet, IL-4and GATA-3in control group did not change (P>0.05)2.2.2The tissue remodeling The eosinophil cells, goblet cells and the percentage of extracelluar matrix decreased gradually in treatment group. The contorll group did not change much. Discussion1.The relationship between Jinkui Shenqi pill and allergic rhinitisA lot of schlors presume that allergic rhinitis is related to heredity, surrounding and so on.The symptoms of lung-Qi are sneezing, nasal discharge and nasal ocllusion. But most people of have experienced long course of disease and have recurrent attacks. Kidney is the fundamental of QI and promots inspiring. So Kidney-yang Warming Therapy can cure allergic rhinitis.That is why we choose Jinkui Shenqi pill as a medicine to treat allergic rhinitis2. Relationship between cytokines Th1/Th2and transcription factors T-bet/GATA-3 The disbalance of Thl/Th2differentiation is the key point element of immune response and is reltated with diseases nosogenesis, development and turnover. Transcription factors T-bet/GATA-3can control the differentiation of cytokine Thl/Th2separately. Our study finds out that Kidney-yang Warming Therapy can not only increase Thl and T-bet but also decrease Th2and GATA-3. This may be the mechanisms how Kidney-yang Warming Therapy heal AR.3. Nastal tissue remodeling in allergic rhinitisTissue remodling is the result of repairment to damage. Tissue specificity and disease specificity are different and the change of respairy tract is obviously. Since allergic rhinitis is a disease of respairy tract then there maybe tissue remodling.The purpose of our study is to find out this change of AR before and after treatment. In our experiment allergic rhinitis models showed significant pathological changes in the nasal mucosa,which included eosinophils infiltration into epithelium and submucosa, significant goblet hyperplalsia, epithelial damage and deposition of extracellular matrix. After allergic rhinists model guinea pigs were treated with Jinkui Shenqi pills,these changes were significantly inhibited, in which group, there were fewer eosinophils, fewer goblet cells, relative intact epithelium, and fewer deposition of extracellular matrix. The pathological change can be reversed after8weeks treatment but there were still pathological changes even after12weeks treatment.We should point out that the Chinese herbal medicine is composed by the compound prescribe, implying various valid compositions, for this reason Chinese herbal medicine curing this disease is very complex。Conclusion:1. Kidney-yang Warming Therapy can not only increase Thl and T-bet but also decrease Th2and GATA-3. That is may be the mechanism how it treats allergic rhinitis.2. In our experiment allergic rhinitis models showed significant pathological changes in the nasal mucosa, which included eosinophils infiltration into epithelium and submucosa, significant goblet hyperplalsia, epithelial damage and deposition of extracellular matrix. We still do not know the relationship between pathological changes and clinic symptoms. But as we know more about pathological changes we will learn more about the treatment of AR. In the future the reverse of pathological changes in the nasal mucosa may be new way to cure AR.3. Te Chinese herbal medicine is composed by the compound prescribe, implying various valid compositions. The regulation of T-bet/GATA-3expression may be a key point for Kidney-yang Warming Therapy to cur allergic rhinitis.
