Based On Brain-gut Peptide To Explore The Biological Mechanism Of Functional Diarrhea And Spleen Deficiency Syndrome "spleen Loss And Health" And The Efficacy Mechanism Of Traditional Chinese Medicine

ObjectiveTo explore spleen deficiency mechanism of traditional Chinese medicine,and further improve the spleen deficiency of syndrome essence of TCM,this paper discusses the func-tional diarrhea spleen deficiency and the treatment mechanism of Shenling Baizhu San,and explores the target of disease treatment,based on brain-gut peptide correlation research.MethodsLiterature research:(1)we first arranged clinical literatures of functional diarrhea,dis-cussed the distribution of functional diarrhea,high frequency of TCM syndro-me types,and statisticed to excavate the core symptoms of disease.(2)Based on above work,we discussed the mechanism of TCM pathogenesis of functional diarrhea.In this study,the prescription and traditional Chinese medicine for the treatment of fun-ctional diarrhea were analyzed,and the rules of TCM medicine were discussed.This research used system evaluation and Meta-analysis method to analyze the curative eff-ect of modification of Shenling Baizhu San,objective analysis effective cure rate,recurrence rate,side effects and safety,to provide relia-ble scientific basis for clinical application.Experimental research:(1)Based on the theory of traditional Chinese medicine,we constructed a model of functional diarrhea spleen deficiency syndrome combined with animal model and evaluated it.We used "high lactose feed with water environment Xiaopingtai legislation" composite factors to establish the animal models,to observe the rats in signs,body weight,diet,and detected the micro indicators(the content of serum amylase,D-xylose and lactic acid),and evaluated animal models of functional diarrhea and spleen deficiency syndrome.(2)After the animal model was successfully established,we used immune-histochemistry,Elisa,Real-time PCR and Western blot to detect the brain-gut peptide(Ghrelin,CCK,and VIP)in the hypothalamus and serum of rats.(3)In addition,the model rats were treated with Shenling Baizhu San and the western medicine smecta as the positive reference drug.During the period,recorded the diet of rats,the diarrhea index,and used the micro indicators(the content of serum amylase,D-xylose and lactic acid)to evaluate effect of drug treatment,(4)We used immunohistochemistry,Elisa and Real-time PCR,Western blot to detect the brain-gut peptide(Ghrelin,CCK,VIP)in the hypothalamus and serum of rats.ResultsLiterature research results showed:(1)The high frequency of TCM syndrome types in the order:weakness of spleen and stomach,weakness of spleen and kidney,stagnation of liver qi and spleen deficiency,cold heat complication,spleen asthenia and dampness retention,com-mon symptoms were diarrhea(The defecation increases,the nature is clear thin or loose.),the disease location was mainly in the spleen,stoma-ch,liver and kidney,main responsibility was to the spleen.This disease was due to the deficiency of spleen and the moist turbidity,which leads to gastrointestinal dysfunct-ion,and the pathogenic factors such as phlegm,drinking,water,and humidity had been stopped in the abdomen.(2)In the clinical treatment prescri-ptions,it is mainly used to invigorate the spleen,to collect the lung,and to gasify and humi-dify.There is a high degree of consistency between the highest frequency of traditional Chi-nese medicine and the traditional Chinese medicine,Shenling Baizhu San.Therefore,it can be used as the main prescription of the disease.This also confirmed the correlate on be-tween functional diarrhea and spleen deficiency from the perspective of treatm-ent.(3)We conducted systematic evaluation and Meta-analysis of RCT literatures on the treatment of functional diarrhea by Shenling Baizhu San.It is found that the comprehensive curative effect of Shenling Baizhu San on functional diarrhea is better than that of western medi-cine,which can further reduce the recurrence rate and high safety.Experimental research results showed:(1)we established "high lactose feed with water environment Xiaopingtai legislation" compound factor model rats,which had significantly higher diarrhea index than normal group rats.Model rats showed signs of fatigue,lack of luster,decreased diet and other physical signs,while serum amylase and D-xylose contents were significantly reduced,serum lactate content increased significantly.These characteristics meet the criteria of functional diarrhea spleen defi-ciency syndrome.(2)In terms of biological indicators,Ghrelin and CCK increase-ed the expression level of protein and mRNA in the hypothalamus of rats with functional diarrhea spleen deficiency syndrome,and increased in circulating blood.The standard of VIP in hypothalamus and circulating blood decreased,and the protein was consistent with mRNA trend.Brain-gut peptide disorder may be the patholo-gical mechanism of functional diarrhea,which caused the abnormal movement of gastroin-testinal function and the decrease of digestion and absorption,and induced diarrhea of "dys-function of the spleen in transport".(3)On model animal medication,the study found that the spleen and three prescriptions Shenling Baizhu San could significantly improve the con-tent of serum amylase and d-xylose,improve gastrointestinal func-tion,reduce the content of lactic acid,and relieve the symptoms of spleen defici-ency.It is proved that Shenling Baizhu San has definite therapeutic effect on sple-en deficiency syndrome of functional diarrhea.(4)Shenling Baizhu San can reduce the content of Ghrelin and CCK in the hypothalamus and peripheral blood and incr-eased the content of VIP.It is found that the therapeutic mecha-nism of Shenling Baizhu San for the spleen deficiency syndrome of functional diarrhea may improve the related symptoms by regulating the expression of brain-gut peptides.The study also indicated that the therapeutic effect of Shenling Baizhu San on functional diarrhea spleen deficiency syndrome was related to the regulation of the expression of brain-gut peptides.ConclusionsTo sum up,functional diarrhea belongs to the category of "diarrhea" in Chinese medi-cine,which is closely related to spleen deficiency syndrome.Shenling Baizhu San is an effective prescription for the clinical treatment of the disease,which has a significant effect on the treatment of western medicine.Experimental study confirmed that involved three atracty-lodes powder can obviously improve the symptoms and signs of functional diarrhea spleen deficiency rats,reducing frequency of defecation,stool change character,improve digestion and absorption ability,alleviate fatigue state.Biological mechanism study showed that func-tional diarrhea spleen deficiency model rat hypothalamus and Ghrelin,CCK expression in peripheral blood increases,the VIP express,suggests that the pathogenesis of functional dia-rrhea spleen deficien-cy may for the abnormal expression of brain-gut peptides to induce intestinal home-ostasis and gastrointestinal dysfunction.After the treatment of Shenling Baizhu San,the expression of Ghrelin,CCK and VIP in the hypothalamus and peripheral blood of rats with functional diarrhea syndrome were normal.These results suggest that Shenling powder may regulate the expression of brain intestinal peptide,thus maintaining neuroendocrine network homeostasis and regulating gastrointestinal function.The study also further indicates that Ghrelin,CCK and VIP may be the biological specificity index and effective therapeutic target of functional diarrhea spleen deficie-ncy syndrome.
