Study On The Dosage And Related Research Of Chinese Medical Masters Using Eliminating Stasis Drugs Based On Data Mining

Objective:To explore the dosage and related rules of clinical application of Eliminating stasis drugs by Traditional Chinese Medicine Masters.These rules can provide guidance for the clinical safe and effective use of drugs,can provide an objective basis for the national authorities to improve the relevant dose regulations,and also can provide some data support for the further study of drugs.Methods:Collect the medical records and words published by the Master of Traditional Chinese Medicine,and set up the"Database of Chinese Medical Master"（DB-GYDS）through reasonable inclusion and exclusion criteria.Search according to the drug name,extract the prescriptions of Eliminating stasis drugs from the database as data sets,use SPSS23.0 and TIMSS software to mine and analyze the dose application law of drugs,representative drugs（Salvia miltiorrhiza,Chuanxiong,Hirudo,Eupoly-phaga）main disease rules and compatibility rules.Results:1.There are 1766 prescriptions containing Rhizoma Ligustici Chuanxiong,the dose threshold of Rhizoma Ligustici Chuanxiong is 1⁷5g,the average dose is（9.46±5.19）g,P₁₀^P₉₀ is 5¹5g.The most commonly used dose of Chuanxiong is 10g,and the percentage of over dose prescriptions is 19.71%;There are 647 prescriptions containing Rhizoma Corydalis,the dose threshold of Rhizoma Corydalis is 3³0g,the average dose is（12.21±4.39）g,and P₁₀^P₉₀ is 9²0g.The most commonly used dose of Rhizoma Corydalis is 10g.The percentage of over dose prescriptions is 38.33%;There are 1016 prescriptions containing Radix Curcumae,the dose threshold of Radix Curcumae is1.5³2g,the average dose is（11.09±3.80）g,P₁₀^P₉₀ is 9¹5g,the most commonly used dose is 10g.The percentage of over dose prescriptions is 33.07%;There are 217 prescriptions containing Faeces Togopteri,the dose threshold of Faeces Togopteri is 5⁶0g,the average dose is（10.35±5.54）g,P₁₀^P₉₀ is 6¹5g,the most commonly used dose is 10g.The percentage of over dose prescriptions is 17.97%;There are 129 prescriptions containing Olibanum,the dose threshold of Olibanum is 0.5²0g,the average dose is（7.47±3.42）g,P₁₀^P₉₀ is 3¹0g,the most commonly used dose is 10g.The percentage of over dose prescriptions is 70.54%;There are 144 prescriptions containing Myrrh,the dose threshold of Myrrh is 0.5²0g,the average dose is（7.19±3.43）g,P₁₀^P₉₀ is 3¹0g,the most commonly used dose is 6g.The percentage of over dose prescriptions is 64.58%;There are 126prescriptions containing Lignum Dalbergiae Odoriferae,the dose threshold of Lignum Dalbergiae Odoriferae is 1.5¹5g,the average dose is（7.16±3.65）g,P₁₀^P₉₀ is 2.4¹2g,the most commonly used dose is10g.For this drug,no more than the prescribed dose is used;There are2282 prescriptions containing Salvia miltiorrhiza,the dose threshold of Salvia miltiorrhiza is 1.5⁵0g,the average dose is（18.03±6.87）g,P₁₀^P₉₀ is 10³0g,the most commonly used dose of Salvia miltiorrhiza is15g.The percentage of over dose prescriptions is 39.70%;There are 995prescriptions containing Flos Carthami,the dose threshold of Flos Carthami is 1³0g,the average dose is（9.42±3.31）g,P₁₀^P₉₀ is 6¹5g,the most commonly used dose is 9g.The percentage of over dose prescriptions is 17.19%;There are 1236 prescriptions containing Semen Pers-icae,the dose threshold of Semen Persicae is 3³0g,the average dose is（11.02±3.46）g,P₁₀^P₉₀ is 9¹5g.The most commonly used dose of Semen Persicae is 10g,and the percen-tage of over dose prescriptions is 29.85%;There are 554 prescriptions containing Herba Leonuri,the dose threshold of Herba Leonuri is 6¹20g,the average dose is（19.60±14.23）g,P₁₀^P₉₀ is 9³0g.The most commonly used dose of Herba Leonuri is 15g,and the percentage of over dose prescriptions is 3.43%;There are 293 prescriptions containing Herba Lycopi,the dose threshold of Herba Lycopi is 2⁵0g,the average dose is（12.39±5.01）g,P₁₀^P₉₀ is 9¹5g.The most commonly used dose of Herba Lycopi is 10g,and the percentage of over dose prescriptions is38.23%;There are 375 prescriptions containing Caulis Spatholobi,the dose threshold of Caulis Spatholobi is 6⁵0g,the average dose is（20.66±7.63）g,P₁₀^P₉₀ is 12³0g.The most commonly used dose of Caulis Spatholobi is 15g,and the percentage of over dose prescriptions is53.87%;There are 167 prescriptions containing Eupolyphaga Seu Steleophaga,the dose threshold of Eupolyphaga Seu Steleophaga is 3³0g,the average dose is（7.89±3.03）g,P₁₀^P₉₀ is 5¹0g.The most commonly used dose of Eupolyphaga Seu Steleophaga is 10g,and the percentage of over dose prescriptions is 5.39%;There are 324prescrip-tions containing Zedoary turmeric,the dose threshold of Zedoary turmeric is 3³0g,the average dose is（11.28±4.47）g,P10^P90 is 6¹5g.The most commonly used dose of Zedoary turmeric is 10g,and the percentage of over dose prescriptions is 34.57%;There are 129prescriptions containing Rhizoma Sparganii,the dose threshold of Rhizoma Sparganii is 4²0g,the average dose is（10.60±3.43）g,P₁₀^P₉₀ is 6¹5g.The most commonly used dose of Rhizoma Sparganii is10g,and the percentage of over dose prescriptions is 31.78%;There are220 prescriptions containing Hirudo,the dose threshold of Hirudo is 1³0g,the average dose is（5.62±3.68）g,P₁₀^P₉₀ is 3¹0g.The most commonly used dose of Hirudo is 3g,and the percentage of over dose prescriptions is 57.73%;There are 247 prescriptions containing Squama Manis,the dose threshold of Squama Manis is 3⁴5g,the average dose is（9.94±4.56）g,P10^P90 is 6¹5g.The most commonly used dose of Squama Manis is 10g,and the percentage of over dose prescriptions is13.77%。2.The high frequency diseases treated by Radix Salviae Mil-tiorrhizae are Coronary heart disease,Palpitations and Irregular menstruation.It is often compatible with different kinds of drugs,such as Buxu drugs,Huoxue drugs,Qingre drugs,Lishuishenshi drugs,Xingqi drugs,Jiebiao drugs,Anshen drugs,Huatan drugs and so on.The therapeutic window for the treatment of Coronary heart disease is 9⁵0g.Under the condition of"support≥15%,confidence≥0.6",62 groups of high-frequency drug combinations and 76 rules were obtained.The core drug combinations are"Radix Salvia miltiorrhiza-Rhizoma Ligustici Chuanxiong","Radix Salvia miltiorrhiza-Codonopsis pilosula","Radix Salvia miltiorrhiza-Radix Ophiopogonis"and"Radix Salvia miltiorrhiza-Radix Astragali".The therapeutic window of Salvia miltiorrhiza in the treatment of palpitation is 5⁵0g.Under the condition of"support≥15%,confidence≥0.6",68 groups of high frequency drug combinations and 62rules were obtained.when the support≥35%,the core drug combinations are"Radix Salvia miltiorrhiza-Radix Ophiopogon","Radix Salvia miltiorrhiza-Poria cocos","Radix Salvia miltiorrhiza-licorice","RadixSalvia miltiorrhiza-Guizhi","Radix Salvia miltiorrhiza-Radix Astragali","Radix Salvia miltiorrhiza-Schisandra".The therapeutic window of Salvia miltiorrhiza in the treatment of irregular menstruation is 5³0g.Under the condition of"support≥15%,confidence≥0.6",72 groups and102 rules of high frequency drug combination were obtained.When the support≥35%,the core combinations are"Radix Salvia miltiorrhiza-Chuanxiong","Radix Salvia miltiorrhiza-Poria cocos","Radix Salvia miltiorrhiza-Rhizoma Cyperi"and"Radix Salvia miltiorrhiza-Angelica sinensis-Radix Paeoniae Alba".3.The high frequency diseases treated by Chuanxiong are irregular menstruation,headache and coronary heart disease.It is often compatible with different kinds of drugs,such as Buxu drugs,Huoxue drugs,Jiebiao drugs,Qingre drugs,Xifengzhijing drugs,Xingqi drugs,Lishui-shenshi drugs and so on.The therapeutic window of Chuan-xiong in the treatment of menstrual disease is 3¹8g.Under the condition of"support≥20%,confidence≥0.9",44 groups of high-frequency drug combinations,60 rules were obtained.Under the condition of support≥30%,the core drug combinations are"Chuanxiong-Angelica-Radix Paeoniae Alba","Chuanxiong-Angelica-Cyperus","Chuanxiong-Angelica-Radix Rehman-niae","Chuanxiong-Angelica-Herba Leonuri","Chuanxiong-Angelica-Codonopsis pilosula".The therapeutic window of Chuanxiong in the treatment of headache is 3⁷5g.Under the condition of"support≥15%,confidence=1",47 groups of drug combinations and 34 rules were obtained.Under the condition of support≥25%,the core drugs were obtained as"Chuanxiong-Angelica","Chuanxiong-Salvia miltiorrhiza","Chuanxiong-safflower","Chuanxiong-licorice","Chuanxiong-RadixPaeoniae rubra","Chuanxiong-Asarum","Chuanxiong-Gastrodiaelata","Chuanxiong-Radix Paeoniae Alba","Chuanxiong-Angelicadahurica","Chuanxiong-chrysanthemum".The therapeutic window of Chuanxiong in the treatment of Coronary heart disease is 3³0g.Under the condition of"support≥25%,confidence=1",39 groups of high-frequency drug combinations and 23 rules were obtained.When the support≥40%,the core drug pairs are"Chuanxiong-Salvia miltiorrhiza-Radix Astragali","Chuanxiong-Radix Paeoniae Rubra","Chuanxiong-Angelica"and"Chuanxiong-Ophiopogon".4.The high frequency disease treated by Hirudo is stroke.It is often compatible with different kinds of drugs,such as Huoxue drugs,Buxu drugs,Qingre drugs,Xifengzhijing drugs,Lishuishenshi drugs,Jiebiao drugs,Qufengshi drugs,Huatan drugs,Zhixue Xiexia drugs,and so on.The therapeutic window of leech for stroke is 1²0g.Under the condition of"support≥20%,confidence≥0.6",60 groups of high frequency drug combinations and 107 rules were obtained.Under the condition of support≥40%,the core drugs for the treatment of stroke are"Hirudo-Chuanxiong","Hirudo-LumbriCus","Hirudo-Semen Persicae","Hirudo-salvia miltiorrhiza","Hirudo-Radix Paeoniae Rubra".5.The high frequency disease of Eupolyphaga treatment is Rheumatic arthralgia and Cancer.It is often combined with Tonic drugs,Eliminating stasis drugs,Heat clearing drugs,Xifeng antispasmodic drugs,etc.The therapeutic window of rheumatism Bi disease was 4.5¹5g.Under the condition of"support≥20%,confidence≥0.6",36 groups of high-frequency drug combinations and 45 rules were obtained.When the support≥30%,5 groups of core drug combinations were obtained:Eupolyphaga-Angelica licorice,Eupolyphaga-Radix Aconiti,Eupolyphaga-Ramulus Cinnamomi,Eupolyphaga-Caulis Caulis bugui-Clematis chinensis.The treatment window of Eupolyphaga for Cancer is 5¹5g.Under the condition of"support≥30%,confidence=1",29 groups of high-frequency drug combinations were obtained and 25 rules were obtained.When the support was greater than 50%,we can find that there are 3 core drug combinations which are Eupolyphaga-Herba Hedyotis,Eupolyphaga-Herba Scutellar-Iae Barbatae,and Eupolyphaga-Rhizoma CurcumaeConclusion:The flexibility of the dose threshold range of Traditional Chinese Medicine Masters in the application of Eliminating stasis drugs is generally large,the average dose,P₉₀ and the upper limit of dose threshold are more than the quantitative range specified in the Pharmacopoeia.The application of Chinese medicine masters to the dosage of Eliminating stasis drugs is not completely restricted by the rigidity of the legal dose in Chinese Pharmacopoeia,and the over-use has become a norm.To a certain extent,the dosage application of Chinese medicine masters to Eliminating stasis drugs reflects the limitations of the legal dosage of Chinese Pharmacopoeia.Salvia miltiorrhiza,Chuanxiong Leech and Eupolyphaga have different treatment windows for different diseases.If the treatment window is different,the efficacy of drugs is also different.The dose application of medicine by the master of Chinese Medicine can be summarized as a Disease-Certificate-Physique-Medicine model.The four kinds of drugs are widely used in the clinical treatment of diseases,and there are different rules of compatibility for different diseases and syndromes.The medicine compatibility can be summarized as the principle of Disease-Certificate-medicine.These rules are a high level of summary of the clinical experience of Chinese medicine masters.They have important theoretical and clinical application values,and are worthy of our indepth research and study.
