Based On Data Mining,the Academic Thought And Medication Law Research Of Zhang Lei,a Master Of Traditional Chinese Medicine,in The Treatment Of Paralysis

Objective: By collecting and sorting out the cases of Professor Zhang Lei in the treatment of paralysis,and summarizing Zhang Lao’s medication experience in the treatment of paralysis,in order to better inherit his academic ideas,it provides reference for the clinical treatment of paralysis.Methods: 1.This study collected and sorted out the outpatient cases preserved by Zhang Lei’s famous doctor studio in the Third Affiliated Hospital of Henan University of Chinese Medicine from January 2018 to August 2022,and consulted the publicly published monographs: "Zhang Lei Medical Library of Master of Traditional Chinese Medicine","Zhang Lei Medical Encyclopedia","Zhang Lei’s Evidence Collection",a total of 550cases(708 consultations)of complete medical cases were sorted out,and an Excel database was established.2.Microsoft Office Excel 2021 was used for frequency statistical analysis,R Version 4.2.1 for group association rule analysis,and SPSS Statistics 26.0 for traditional Chinese medicine factor analysis and cluster analysis.3.Based on the data mining results,combined with Professor Zhang Lei’s actual clinical experience and medication habits,analyze the medication rules of related medical cases,and briefly analyze Professor Zhang Lei’s academic ideas on the treatment of paralysis.Results: 1.Analysis of 550 patients,there were more women than men in terms of gender,and the age was mainly distributed in 41～50 years old;Core symptoms are pain,morning stiffness,swelling,fatigue;The main tongue image is red tongue,moss thin white;The main vein is pulse stagnation;The main symptoms are: wind-cold-dampness arthralgia,syndrome of heat transformed from wind-dampness,Qi and blood deficiency of arthralgia syndrome,Obstruction of phlegm and blood stasis syndrome,liver and kidney deficiency.A total of 780 prescriptionos were collected,involving 347 flavors of traditional Chinese medicine,with a total frequency of 8838 medications,and the top 20 drugs treated by Professor Zhang Lei were Radix Paeoniae Alba,Radix Glycyrrhizae,Radix Angelicae Sinensis,Ramulus Cinnamomi,Hedysarum polybotrys Hand,Semen Coicis,Radix Clematidis,Praeparata Cum Melle,Rhizoma Ligustici Chuanxiong,Rhizoma Atractylodis Macrocephalae,Radix Saposhnikoviae,Poria,Rhizoma Zingiberis Recens,Radix Angelicae Pubescentis,Radix et Rhizome Achyranthes,Radix Aconiti Lateralis Preparata,Rhizoma Atractylodis,Fructus Jujubae,Radix Rehmaniae Recens,and Caulis Spatholobi.According to the statistics of the nature and flavour of drugs,the warm,cold,flat drugs were the most frequently used.Bitter,sweet and sinuous drugs are the main medicinal tastes;In the channel entry,it is mainly attributed to the liver channel,kidney channel and spleen channel.The most common drug effects are antipyretic drugs,deficiency medicines,rheumatic drugs,water and moisture drugs,and blood circulation and stasis removal drugs.Association rule analysis: The second,third and fourth order association rule analysis was carried out on 374 flavors of traditional Chinese medicine,and a total of 58 rules were produced,and it can be summarized from the association rule bubble chart that from the perspective of support,{Ramulus Cinnamomi}=>{Radix Paeoniae Alba},{Radix Paeoniae Alba,Hedysarum polybotrys Hand}=>{Ramulus Cinnamomi} and {Radix Paeoniae Alba,Fructus Jujubae,Rhizoma Zingiberis Recens}=>{Ramulus Cinnamomi} were the combination of two,three and four drug association rules with the highest degree of support In terms of confidence.{Herba Asari}=>{Ramulus Cinnamomi},{Radix Paeoniae Alba,Fructus Jujubae}=>{Ramulus Cinnamomi} and {Radix Paeoniae Alba,Fructus Jujubae,Rhizoma Zingiberis Recens}=>{Ramulus Cinnamomi} were the highest confidence combinations of two,three and four drug association rules.Traditional Chinese Medicine(TCM)Factor Analysis: The first 25 flavors of high-frequency drugs were factorically analyzed,and a total of 9common factors were extracted,including 2 single flavor Chinese medicine common factors,3 drug pairs and 4 drug groups.Traditional Chinese Medicine(TCM)Cluster Analysis: The top 30 Chinese medicines were clustered to obtain 5 cluster combinations,including 1 self-formulated formula and 3 addition and subtraction groups.2.Professor Zhang Lei’s academic thought on the treatment of paralysis,from the perspective of etiology and pathogenesis,emphasizes the false standard,internal and external causes of paralysis;In terms of differentiation,it emphasizes cold and heat,virtual reality,attaches importance to the combination of disease discrimination and differentiation,pays attention to the identification of primary and secondary diseases,embodies the idea of treating diseases before they occur,and prevents changes from existing diseases;In terms of governance and treatment,it is mainly to harmonize camp guards,care for medium focus,promote qi circulation,activate blood circulation and remove stasis,and nourish liver and kidney;The characteristics of the drug are small and specialized,the use of medicine is mainly light and spiritual,and the good use of black attachment,insects,and rattan drugs to treat paralysis.Conclusion: 1.Through the analysis found that Professor Zhang Lei in the treatment of arthralgia on the experience of rich,send prescription medication,to and as the method,to flat period,small amount and special,drug light spirit.2.Starting from "syndrome of lingering pathogen due to deficient vital qi",Zhang Lao believed that "vital qi deficiency" was the root of the disease,taking "harmonizing yingfen and weifen" as the program,focusing on the three methods of "harmonizing method","obstruction-removing method" and "tonifying method",combining disease differentiation with syndrome differentiation,paying attention to the main and secondary symptoms,grasping the pathogenesis,and increasing and decreasing with the disease.3.Huangqi Guizhi Wuwu Decoction was commonly used for clinical syndrome.In addition to routine dispelling wind and dampness,channeling meridians,promoting blood circulation,removing blood stasis and relieving pain,the importance of taking care of the spleen and stomach is emphasized.While strengthening the spleen and qi,it is also used to remove dampness.4.Drug pair commonly used Ramulus Cinnamomi with Radix Paeoniae Alba harmonizing yingfen and weifen,Semen Persicae with Flos Carthami to promoting blood circulation for removing blood stasis;wind-cold-dampness arthralgia commonly used Juanbi decoction to reduce;arthralgia of heat type commonly used ermiao powder addition or reduction;Xuefu Zhuyu decoction is commonly used to reduce blood stasis;Channels and collaterals not smooth commonly used "Shuli method" treatment,the Self-fictional formula is Xing Qi Tongluo Fang,phlegm Tongluo Fang,Huayu Tongluo Fang.
