| Objective:Build a community TCM health management system based on TCM status science,and discuss the promotion experience.Methods:1.Literature review and research,through the government website,VIP database to find the policy documents and literature on the community TCM health management,the analysis of foreign and domestic health management status,put forward the existing problems;2.System construction,and build a TCM health management system based on TCM status science community;3.Empirical research and demonstration application,apply the system to 11 community health service centers in Gulou District for empirical research,and demonstrate it from the "morbid" level,randomly sampling elderly hypertension patients,divided into control group and observation group;applying the old and new system to evaluate the health management.Results:1.The research on the theory,model and practice of health management at home and abroad has been continuously deepened,but there are still problems in the top-level design,theoretical system and methodology,and model application.2.With the support of the policy,a TCM health management system based on the application of new technologies and new models is established based on the existing community health service system.3.Empirical study,44 out of 50 indicators in 3 categories of 11 community health service centers in Gulou District,Fuzhou from January 2016 to December 2020 achieved positive growth,with 36 increasing by more than 10%.Demonstration and application,This experiment included 224 cases,and the distribution of vertigo names was: 58.9%,12.5%,bi,6.3%,headache,2.7%,2.7%,1.8%,edema,1.3%and chest pain,0.9%.The distribution of disease position is: liver composition ratio is 32.3%,kidney composition ratio 29.5%,heart composition ratio 17.6%,spleen composition ratio14%,lung composition ratio 4.3%,and gastric composition ratio 2.3%.Pathological distribution is: Yin deficiency composition ratio is 26%,17.9%,14.4%,11%,blood stasis composition ratio is 9.7%,8.5%,2.5%,2.2%,and heat composition ratio is 2.2%.Blood pressureIn terms of the final blood pressure control effect,compared with the mean systolic blood pressure between the control group and the observation group,and the systolic blood pressure decreased more significantly in the observation group.In the control group,systolic and diastolic blood pressure decreased significantly in health management,which only decreased significantly in the middle stage,and the positive relationship between intervention effect and courses was not obvious.In the observation group,systolic blood pressure decreased significantly after TCM health management,and a positive relationship between intervention effect and treatment course was obvious.Diastolic blood pressure decreased significantly,which decreased significantly only in the middle stage,and the positive relationship between intervention effect and courses was not obvious.SymptomsIn terms of the final symptom control effect,the symptom mean of the control group was compared with the observation group,and the symptom degree decreased more significantly in the observation group.In the control group,the improvement effect of the symptoms after health management was obvious,which was only obvious in the middle stage,and the positive relationship between the intervention effect and the treatment courses was not obvious.In the observation group,the symptom improvement effect after TCM health management was obvious,and the positive relationship between the intervention effect and the treatment course was obvious.Quality of life(SF36)In terms of the improvement of the final quality of life,the degree of SF36 increased more significantly in the end control group and the observation group,compared with the mean value of SF36 in the observation group.In the control group,the improvement effect of SF36 after kang management was obvious,which was only obvious in the middle stage,and the positive relationship between the intervention effect and the courses was not obvious.In the observation group,SF36 improved significantly after health management,and the positive relationship between intervention effect and courses was obvious.Conclusion:The hospital-style TCM health management system based on community family doctor gatekeepers in the state of TCM constructed in this study has clearer policy objectives and implementation paths,more complete theoretical system and methodology,and more operational base application template.Research on the "morbid" level of TCM health management in the elderly with hypertension shows that the main vulnerable factors of hypertension,Yin deficiency,qi deficiency,phlegm and other susceptible factors,are closely related,and the disease position is mainly in the liver,kidney,heart and spleen.The main disease name is vertigo,which is consistent with literature research,which is conducive to risk warning.TCM health management can effectively control the blood pressure level of hypertension,improve the uncomfortable symptoms and quality of life of hypertension,and have a two-way regulation effect.Meanwhile,TCM conditions have the overall characteristics,and the regulation effect gradually works over time.The established community TCM health management system built is more efficient. |
