Investigation On HIV Infection Status Among Some Rural Migrant Women In Shandong Province

Objective To investigate the rate of HIV infection, high risk factors and knowledge, attitude and behavior of HIV among non-local-women came from other provinces and autonomous region (Yunnan and Guangxi) out of Shangdong province and foreign areas (Vietnan and Burma) who married with local people in the countrysides of Shandong province. Moreover, risk factors in related to the HIV infection as well as the rate of second generation transmission were revealed.Methods1. Three counties in Shandong province where the residence of non-local-women was relatively concentrated. The other consideration for selection was the willing in compliance with this survey of the local governments and organizations. Cross-sectional investigation was adapted for non-local-women and50%for questionnaire within them.2. HIV-positive non-local-women from cross-sectional field and some HIV-positive non-local-women picked from Qingzhou city, Shandong province, in2004were selected as case group. HIV-negative non-local-women were selected as control groups by1:4matching with their age, time of migration and their native place.3. Reason of migration flowing into Shandong province and its trends as well as the incidence of those people were studied by using well-designed questionnaires and interviews.4.The spouses and children of the HIV-positive non-local-women were also investigated.Results1.5HIV positive cases out of2515blood samples were detected. The rate of HIV positive among those women is0.2%(0-0.39%) non-local-women were interviewed.1519non-local-women were interviewed for questionaire. All of the HIV-positive non-local-women were come from serious epidemic areas during2000-2005, which account for0.25%(5/1968) of total non-local-women who came from serious epidemic area from1994-2005and account for0.7%(5/638) of those women who came from serious epidemic area during2000-2005.2. Multivariate logistic regressions analysis showed that the risk factors associated with HIV affection among non-local-women were marital history in home of original residence place (OR=14.0,95%CI:3.67-56.75). The rates of attribute risk were92.8%.3. Routes of infection among18HIV-positive migration women are sexual intercourse (61.1%), sexual intercourse or sharing syringes for drug abuse (16.7%), operational route (5.6%) and unknown routes (16.7%).4. For second generation transmission, vertical transmission was8.3%(1/12),and spouse transmission was27.8%(5/18).5. The rate of understanding knowledge of AIDS among non-local-women was36.8%.6. The factors of women flowed into county of Shandong province are economy, traffic and culture, ect. Total1519women migrated from1994-2005period were investigated. The distribution were546during1990-1994(35.9%),472during1995-1999(31.1%) and501during2000-2005(33.0%).7. The rates of unprotected sexual behavior and drug abuse among migrated women were13.1%and0respectively.Conclusion1. The rate of HIV infection was high in this survey.2. The risk factors in related with HIV infection among non-local-women was marital history in original home place home.3. The migrated women came from serious HIV infectious areas caused the second generation infections in Shandong province.4. Poor knowledge in the field of AIDS among non-local-women, and the great number of the population movement especially women from HIV epidemic areas with high-risk behaviors contributed increasing risk factors in Shandong Province.
