Background Semen is mainly composed of sperm and seminal plasma which accounts for about 90%. Sperm is produced by testis. It travels and stores in epididymis. Afterwards, sperm develops into the mature sperm and obtains motor ability. Ejaculation is completed after sperm is emitted into spermaduct. Sperm turns mature in seminal plasma which supplies most energy for the motion of sperm. In addition, a certain component in seminal plasma such as decapacitation factors, maturation factors in epididymis makes impact on the function of sperm. Seminal plasma is closely related to the function, survival and motion of sperm, which might influence human reproduction. Different components in seminal plasma play a pivotal role in the development and maturation of sperm. In the meanwhile, the component content in seminal plasma can reveal the function status of glands concerned. AMH is the result of cell secretion by immature sustentacular cells in testis. It can adjust the cell differentitation, the gonad of development and the decline of testicular. In this regard, AMH might have impact on male reproduction. At home and abroad, plenty of studies have been carried on AMH, the intensity and sperm motility of sperm. However, they did not shed light on the mechanism of male reproduction with a lack of studies on the relationship between AMH and male infertility. What’s more, there is sparse literature about the function of AMH from the perspective of traditional Chinese medicine. Objective There exist many problems in the studies on AMH in the fields of traditional Chinese and western medicine. With the guidance of my supervisor, the author designed the research so as to unfold the relation between AMH and the mass parameter of semen plus infertility. Besides, the thesis will elaborate the impact of Qilin Wan, a type of traditional Chinese medicine, on AMH.Method The samplers,97 in total, were from the male patients in urology department in East Hospital of Beijing University of Traditional Chinese Medicine. The author collected all the information including names, ages genders, diagnosis history, previous anamnesis, physical examination, auxiliary examination.The 97 samplers were divided into two groups:Infertile Group including 56 cases and Normal Group with Normal Pregnancy Test Result including 41 cases. For each group member, at the very beginning of experiment, he would receive two types of test:routine semen analysis and sperm morphology analysis. Then through centrifugation of semen, the intensity of seminal plasma would be given afterwards.For the Infertile Group, the author tested their serum sex hormone including FSH, LH, T, E2, and PRL. For the Normal Group,23 patients with deficient kidney essence would be given Qilin Wan. After three months, they would be given semen analysis, sperm morphology analysis and intensity analysis of AMH.In the design, the author unfolds the relation between AMH and mass parameter of semen. By means of comparison of the intensity, semen analysis results, sperm morphology of the two groups, the author observes significant differences of the two groups. For the Goup Two, the author analyzes the relation among AMH, semen analysis results, serum sex hormone and made a comparison of intensity of AMH, semen analysis and seminal plasma analysis between the group with use of Qilin Wan and the group without Qilin Wan.Result It is found that there is no striking difference in aspect of ages and abstinence between the Group of Infertility and the Normal Group. Additionally, in the two groups, AMH is positively associated (P<0.01) with rate of A-level sperm, A&B-level sperm of sperm and is unrelated to morphology with no difference in the result of AMH. Moreover, sperm motility in Infertile Group is weaker.In Infertile Group, plasma AMH in patients with normal Sperm motility was significantly higher than that of asthenospermia patients (P= 0.01); there is no significant differences in plasma AMH between normal morphology and abnormal; there is no significant correlation between seminal plasma AMH and sex hormone. QIlin wan can obviously improve seminal plasma AMH (P< 0.05) and A-level sperm (P< 0.01).Conclusion AMH is positively associated with rate of A-level sperm, A&B-level sperm. As the sperm analysis result is improved, AMH is promoted without any relation with infertility. This can be the effectiveness index of infertility diagonosis. Furthermore, it is found that Qilin Wan can effectively improve the rate of A-level sperm and the intensity of AMH of the infertile.
