Objective: l.To measure the levels of basal serum follicle stimulating hormone (FSH), luteinizing hormone (LH), estradio(E2) in peripheral blood in natural menstrual cycle and to investigate the correlations between the hormones and IVF-ET outcome. 2.To measure the levels of FSH LH E2 in peripheral blood in controlled ovarian hyperstimulation(COH) cycle and to investigate the correlations between the hormones and IVF-ET outcome. 3.To measure the concentrations of FSH in follicular fluid (FF) in COH and the ratio of FF and serum FSH to research the correlations between them and IVF-ET outcome. 4. To measure the expression of follicle stimulating hormone receptor (FSH-R) on cultured granrlosa cells and to study the correlation between the expression and IVF-ET outcome; to investigate the physiological mechanism of reactivity of the ovary to productive hormones in order to give a rich theoretic base on COH.Methods: 38 cases (38 cycles) including 30 cases of sterility with tubal factor and 8 cases of sterility withunknown reasons were studied in the aspect of FVF-ET in our reproductive center. The sterility caused by male factors were excluded form this study. 1.Natural menstrual cycle: Blood samples were collected on the day 3 in the natural menstrual cycle before the COH cycle. FSH, LH, E2 were measured. 2.COH cycle: Blood samples were collected on the day of FSH-HP administration, hCG administration, oocyte retrieval. Serum were stored at -20 for measuring FSH, LH ,E2. In each case .according to the routine , on the day of oocyte retrieval all follicles were retrieved, and the FF was centrifuged and the supernatant liquid was stored at -20 for measuring FSH. After fertilization granulosa cells and oocytes were isolated and continued to be cultured for 48 hours, and made into suspension; the levels of FSH-R on the the granulose cells shall be immediately measured.Results: 1. The levels of mean basal FSH on natural menstrual cycle on the day 3 in the non-pregnant cycles was higher than that in the pregnant cycles (P0.05), the levels of them correlated negatively with the numbers of oocyte ,the ratio of fertilization and cleavage (r=0.63 PO.001, r=0.43 P<0.05, r=0.46 P<0.05 respectively), but had no significant relationship with expression of FSH-R(P>0.05). The levels of E2 on the same day correlated negatively with the numbers of oocyte(r=0.41 P0.05). The levels of LH, E2 on the same day had no difference in the pregnant cycles and non-pregnant cycles and had no significant relationship with expression ofFSH-R (P>0.05). 2.The levels of serum FSH, LH in peripheral blood on the day of FSH-HP administration, hCG administration, oocyte retrieval had no significant difference in pregnant cycles and non-pregnant cycles(P>0.05), and had no relationship with the expression of FSH-R and the ratio of fertilization and cleavage (P>0.05). The levels of serum E2 on the day of hCG administration correlated positively with the numbers of oocyte(r=0.43 P<0.05). 3.The level of FSH in FF and the ratio of FSH in the pregnant cycles was higher than that in the non-pregnant cycles (P<0.01), and had positively correlation with the expression of FSH-R(r=0.49 P<0.05,r=0.59 P<0.01 respectively), and had positively correlation with the ratio of fertilization and cleavage(r=0.58 P<0.01,r=0.62 P<0.01; r=0.62 P<0.01,r=0.63 PO.01 respectively). 4.The expression of FSH-R had no correlation with the numbers of oocyte (P>0.05), but had positively correlation with the mature oocytes(P<0.05). The ratio of fertilization and cleavage had positively correlation with the expression of FSH-R(r=0.57 P<0.01, r=0.61 PO.01 respectively). S.The expression of FSH-R on granulosa cells had significant difference between the pregnant cycles and non-pregnant cycles.
