objective To investigate the effects and related mechanism of ulinastatin during ischemia-reperfusion injury (I-RI) associated with pancreas transplantation in rabbits. Method The rabbit model of orthotropic partial pancreas transplantation was used .The change of malonaldehyde(MDA) and the superoxide dismutase (SOD) and amylase in serum were determined after 3hr 6hr 24hr of reperfusion. The change of histology in graft were dectected by H&E stain. Respectively , Ulinastatin and ulinastatin plus ATP+MgCl2 were used . The influence of ulinastatin and ulinastatin plus ATP+MgCl2 were observed . Rabbits cut spleen only were used as control. Result There were significant of increase in the level of MDA and decrease in the activity of SOD at 3hr 6hr 24hr after reperfusion contrested to the group of contrast group (P<0.01). And the MDA level in the group of ulinastatin were significantly decrease at each time point to the transplantation group(P<0.05),But there were no significant difference in the activity of SOD. The difference was significant at each time point in the group of simply transplantation and ulinastatin plus ATP+MgCl2(P<0.01),there were significant difference at each time point in the activity of SOD between the group of ulinastatin and ulinastatin plus ATP+MgCl2(P<0.05).The amylase level in the groups of ulinastatin and ulinastatin plus ATP+MgCl2 were significantly increase at each time point to the transplantationgroup. Histomorphology observation: the inflammation was alleviated in the group of ulinastatin and ulinastatin plus ATP+MgCl2 to the simply transplantation. Conclusion ulinastatin could reduce the level of MDA after pancreas transplantation, namely reduce the produce of the free radicle ,there were no influence to the activity of SOD, namely they can't enhance the ability of eliminating free radicle, ulinastatin and ATP+MgCl2 were cooperation , they could reduce the produce of the free radicle and enhance the ability of eliminating free radicle. They have the protective effect on ischemia-reperfusion injury associated with pancreas transplantation in rabbits.
