Observation Of Curative Effect On Treatment Of Cirrhosis Of Liver Complicated By HE

Hepatic encephalopaphy (HE) is a syndrome whichcharacterized in metabolic disorder,the acute disturbance andembarrassment of hepatic function and central nervous systemdisturbance. Most of them is caused by cirrhosis of liver(commonlyby virus cirrhosis of liver) and arterial veins of soemmering andvena systemica shunt operation aiming at improving the highpressure of arterial veins of Soemmering. The clinical condition isserious and the death rate is high.The pathogenesis of HE is unclearby far, and most people regard it as the result of many elements.Recent study shows that high blood ammonia disease plays themain part in the pathogenesis of HE. The synthetic measure is takenin clinical treatment at present, in which to lower the high bloodammonia is the main step in HE treatment.The ammonia in blood completes its metabolism mainly byurea circulation of liver and the synthesis of glutamine. Mostparenchymal liver cells are destroyed in HE. The hepar's ability tosynthesize urea and clear ammonia decreases or disappears, orbecause of the foundation of the arterial veins of Soemmering andvena systemica circulation, ammonia from intestinal tract can't becleared without hepar detoxication. The blood ammnia siltates inpars encephalica by blood and encephalon,which causes HE .Ornithine-Aspartate(OA) is the combination of aspartate andornithine, which can facilitate the synthesis of glutamine, participatethesynthesis of tricarboxylic acid and nucleic acid indirectly, so itcan facilitate the form of hepar cells and the self repair andanagenesis process of hepar cells. Rui Gan is OA praeparatum. It canimprove patients'condition ,lower blood ammnia,decrese the liverdestruction ,so it can cure HE effecttively.In order to observe the curative effect of Rui Gan, fromJanuary 2003 to January 2006 ,totally 49 cases of cirrhosis ofliver complicated by HE were enrolled. Patients were allocated intoRui Gan treatment group 25 cases and control group 24 cases. Thetreatment group received traditional therapy incombination with 2.5gRui Gan, which is added by 5% glucose, intravenous drip twice perday ,continuously 7 days . The control group receiced traditionaltherapy.Record the patients,symptom and physical sign,(includingrestlessness, calculationa bility orientation force,coma etal),consiciousness time,aminopherase, bilirubin , blood ammnia level,kidney function,blood convention, total effective power and deathrate et al,count scores of HE.Result:Rui Gan treatment group couldimprove the patients'symptom and physical sign more obviouslythan the control group and shorten thepatients,consiciousnesstime,decreased aminnopherase,ilirubin and blood ammnia level, totaleffective power and death rate et al,count scores of HE.Conclusive:Rui Gan is the better drugs for treating HE.Discussion:the reasonofRui Gan is better effective for treatmenting cirrhosis of livercomplicated by HE. Rui Gan is Rui Gan is domestic-made OApraeparatum,it can lower blood ammnia effectively. QuorumOrnithine is the criticality enzyme of synthesizeing urea.Itcanincrease the activity of carbamyl transference and carbamoylphosphate synthetase.so it can make ornithine cimbine with ammniaand toxical metabolic product, facilitate the synthesis of urea anddepletion of toxical metabolic product and lower blood ammnia,accordingly treat HE.Aspartate participate the synthesis oftricarboxylic acid and nucleic acid indirectly, provid intermediateproduct of energy metabolism Aspartate participate, increase supplyof energy of hepra cells,and lower aminnopherase rapidly and bloodammnia,intensifying the hepra cells ingest bilirubin,conbine withand eccrisis it.that profit the recovery of the hepra cells.Inaddition ,Aspartate can also participate the metabolism of hepra cells,provid amino acids of requisition in anthropometric.From this artice ,wo can conclude that Rui Gan is the bettereffective drug for treating HE and it is more safety.
