Infertility is the inability to conceive after one years of regular unprotected sexual intercourse, which is a common source of emotional trauma for infertile couples, the most stressful event in their lives and lead to emotional disorder, and psychological stress. In recent years, following development of assisted reproductive technology (ART), especially in vitro fertilization-embryo transfer (IVF-ET), provide the last hope of pregnancy for infertile couples. However, IVF-ET could bring them painfulness and psychological stress on different extent. Studies involved in the relationship between the IVF-ET and psychological responses have been developed for many years abroad. In our country, the studies in this field are confined to discuss general psychological questions and intervention methods, and seldom focus on issues combining with psychological and clinical medicine.The purposes of this study are to investigate the mental status and changes of IVF women, and influential factors, furthermore analyze the relationship of emotional reactions and treatment outcome. The results of this study will be provide theory foundation to explore psychological interference program which could improve patience treatment compliance, decrease the dropping out and finally enhance pregnancy rate.As study group, total of 118 Infertility women who join in IVF-ET treatment were selected from Reproduction Medical Center in Tianjin Central, Hospital of Obstetrics and Gynecology, according to the entry standard. Total of 50 married health women in Tianjin, matching in age,marriage time, and education, were selected at the same time as controlled group. They were investigated with general condition, psychological status and marital quality questionnaires. And then, if the patients go into the IVF treatment cycle, psychological inventories were tested and the samples of their serum cortisol were measured at three test points.Through investigating and analyzing, the results were as follows:1.Comparing general psychological status and marital quality of study group with controlled group, we found:In two groups, there was significant difference in General mean scores of compelling symptom, depression, anxiety, hostility and crankiness factors from SCL-90 (p<0. 05) . There was negative relevance between somatic symptoms and planning time for pregnancy(p<0. 05)in study group. Marital quality of study group with General value 5 factors was higher than that of controlled group. In particularly, marital satisfaction and methods of solving conflicts were better than that of controlled group (p<0.05) .2.Based on the results of controlling psychological and physiological index on down regulation day, an hour before taking ovum and the 12th day after transplanting waiting for blood {3 -HCG, we found:On three test points, females showed depression, but no statistic difference was found. However, means of state anxiety increased gradually, especially there were significant difference between the 12th day after transplanting waiting for blood ï¿¡5-HCG result and down regulation day result (R0. 05). On three test points, serum cortisollevel was significant higher than that of normal reference value (p<0. 05), which was significant higher at an hour before taking ovum and the 12th day after transplanting waiting for blood P -HCG result than that of down regulation day (K0. 05).3.Correlation factors which IVF-ET females psychological status influenced were:Influential factors of general psychological status included: marital quality, culture degree and occupation. Marital satisfaction was significant higher, and psychological status was significant better. Female' psychological status of above junior college degree was significant better than that of below. And meantime, culture degree influenced the method of solving conflicts. ??Higher degree and better solving method??. Occupation that influenced psychological status from mild to severe was clerk, no work, worker, peasant and private owner. Occupation didn' t influence marital quality.4. We divided females into two groups according to the treatment outcome, and compare variables of psychology and physiology. The results demonstrated that strait anxiety was one of mental influential factors on treatment outcome.In conclusion, through study of IVF females' psychological status, the results proved:1. IVF females' psychological status was remarkable severe than that of general bearing females. During treatment cycle , degree of state anxiety on the 12th day after transplanting waiting for blood P -HCG result was serious than before. State anxiety changes slightly in highgroup of strait anxiety, but the changes of state anxiety showed greater in low group of strait anxiety. Marital quality, strait anxiety, age, marital duration, draft pregnancy time, culture degree, causes of infertility and times of treatment could influence their psychological status. The level of strait anxiety could be related to the treatment outcome.This study pointed out that clinic medical and nursing staffers should pay attention to IVF females' psychological status and influencial factors, and provide the support data for clinic psychological interference program adopting in order to decrease the rate of drop out and increase pregnancy rate.
