Objective: To observe the effects on hemodynamics of two target plasma concentrations of remifentanil alone or combined with TCI propofol in patients of thoracic surgery, evaluate mechanism of and efficacy and safety remifentanil target plasma concentration during thoracic surgery. Methods: Forty ASA I or II patients aged 40-60yr and weighed 52-74kg undergoing pulmonary lobectomy or segmentectomy were randomly divided into 2 groups (n=20 each) according to target plasma concentration of remifentanil: group R6 (6ng/ml) and group R8 (8ng/ml) .The patients were premedicated with midazolam 0.05mg·kg-1 and penehyclidine hydrochloride 0.01mg·kg-1 before anesthesia. S-G catheter was placed via right internal jugular vein and the patients were infused Lactated Ringer's Solution 8-10ml·kg-1·h-1 after entering the operating room. Radial artery was cannulated and continuously monitoring mean arterial pressure (MAP), at the same time ECG, pulse blood O2 saturation (SpO2), BIS was monitored. After 2% lidocaine of topic anesthesia, the HemosonicTM 100 transesophageal echodoppler probe was placed through nose in the esophagus at the level of T5-6 vertebra for monitoring cardiac output (CO), stroke volume...
