| Objective:To compare the effect of the treatment of lumbar spondylolisthesis with high and low waist circumference belt,and to discuss the effect of the treatment of lumbar spondylolisthesis by conservative management,to supply the idealistic clinical basis for the treatment of lumbar spondylolisthesis by waist circumference belt.Methods:From March 2006 to March 2008,46 cases of lumbar spondylolisthesis were treated in our hospital(Zhangzhou hospital of TCM),and who all tallied with the criterion that should be subsumed or eliminated.We divided them into two groups randomly with the random number table.Then one group of 32 cases were treated by high waist circumference belt with functional exercise,the other by low waist circumference belt with functional exercise.By self before-after and comparative study,the score of ODI and VAS was collected by questionnaire and clinical examination to evaluate functional recovery;the effect of preventing progression of spondylolisthesis was evaluated by increasing slippage and change of slip angle.By statistics analysing,we compare the clinical effect of diffieret group of age,type, sex by more point of view and more levels to conclude the clinical effect and the factors on effect in the treatment of lumbar spondylolisthesis.Results:1,All the patients were all followed,the score of ODI after treatment:high group was from 40.36±15.67 to 21±5.78 to 11±4.32,low group was from 39.16±16.03 to 18±7.69 to 8±5.36,so there was obviously significant difference between two groups after three months,but there was no difference between two groups after eighteen months.The score of VAS after treatment:high group was from 8.5±0.9 to 2.7±0.05 to 1.1±0.01,low group was from 7.8±1.1 to 1.1±0.12 to 0.3±0.05,so there was obviously significant difference between two groups after three months,but there was no difference between two groups after eighteen months.2,Evaluation of imaging appearances:after three months,high waist circumference belt is much better than low waist circumference belt in preventing progression of isthmic spondylolisthesis;after eighteen months,high waist circumference belt is much better than low waist circumference belt in preventing progression of degenerative spondylolisthesis.3,Prognostic factor:in the short period, high waist circumference belt is much better than low waist circumference belt in isthmic spondylolisthesis;in the long period,high waist circumference belt is better than low waist circumference belt in degenerative spondylolisthesis,low waist circumference belt has no obviously significant difference between two groups of different sex,type,age.4,after three months,there was no difference between two groups in preventing muscular atrophy and complications:after eighteen months, high waist circumference belt is much better than low waist circumference belt in preventing muscular atrophy and complications.Conclusion:1,Functional exercise with high waist circumference is much better than low waist circumference belt in functional recovery of short period.2,According to different type of spondylolisthesis,we should choose different waist circumference belt with functional exercise to prevent progression in the effect of long or short period. |
