ObjectiveTo compare the effects of target-controlled infusion (TCI) of propofol and remifentanil, TCI of propofol and inhalation of nitric oxide and inhalation of fluorinated and nitric oxide anesthesia on the stress response in patients undergoing Laparoscopic Cholecystectomy (LC).Materials and methods1.experiment materialsSelective LC was performed on 90 cases, male 56 cases, female 34 cases.ASA gradeⅠ-Ⅱclass,age 40-60 years.2.methods for grouping and blockBefore the surgery, the patients were randomly divided intoⅠgroup(standerd general anesthesia combined with inhalation of fluorinated and nitric oxide anesthesia),Ⅱgroup (standerd general anesthesia combined with TCI of propofol inhalation of nitric oxide anesthesia),Ⅲgroup (TCI of propofol and remifentanil),with 30 cases in each.3.detect the indexThe AAI,MAP,HR of three group were monitored at seven time point. In addition, the contration of anesthesia drug and rate of postoperative nausea and vomiting(PONY) were monitored.4.Statistical analysisSPSS 13.0 was used to statistic. Results are present as means±s,inter-group comparision using analysis of variance, intra-group comparision using pairing t test, the contration of anesthesia drug,MAP,HR are compared with AAI using analysis of linearity. Evaluation of PONY use Ridit analysis. It has statistical difference when p<0.05.ResultThe MAP and HR ofⅢgroup were similar to TO time point at T2-T6 time point, but in theⅠⅡgroup, The MAP and HR were significantly lower at TO time point than T2-T6 time point(p<0.05).And comparision of intra-group ofⅠⅡgroup,ⅡⅢgroup, difference were significantly (p<0.05).Conclusion1. TCI of propofol and remifentanil can effectively induce stress response during LC,while maintaining a relative stable hemodynamics.2. AAI can achieve requirement of balancing anesthesia.3. TCI of propofol and remifentanil can effectively avoid appearance of PONY.
