Objective Hyperkalemia is one of the dangerous complications though the progression of renal failure. oral administration of traditional Chinese medicine treatment of renal failure, the effects of traditional Chinese medicine to serum potassium concentration are on controversial. How to ensure the efficacy of Chinese medicine and the same time to avoid the occurrence of hyperkalemia is particularly essential. This study on determine-ation of serum potassium concentration of patients with chronic renal failure after taking prescription Jianpi Yishen, then to get the evaluation of serum potassium concentration and safety of this prescription in patients with renal failure. For the clinical use of prescription Jianpi Yishen provide the basis for safety, and compound on explore new research starting point for the impact of serum potassium concentration by oral medicine in traditional Chinese medicine case studies.Method Acording to regularity of Chronic Renal Failure cases in Chinese medicine hospital of Shenzhen City, which were picked and chose in accordance with the strict inclusion and exclusion criteria, divided into two groups by visit order half randomized, odd number for the treatment group, and even for the control group. The Treatment group consist of 13 cases is treated by TCM which is based on JianPi Yishen, while The control group that 12 cases were collected were intervened by Benazepril Hydrochloride 10mg once a day at eight o'clock. Patients with non-potassium diet, the low potassium diet washout period 1-2 weeks would be required before they join the groups. Clinical manifestations and laboratory indicators of the cases were observed at the begin of the next weekend.Result 1) The change of main indicators of Laboratory analysis showed that, into the group, the treatment group and control group in 24h urinary potassium quantitative, creatinine clearance, creatinine, serum potassium, etc. There was no statistically significant difference (P>0.05); 2) After 1 weeks'treatment period between the two groups, serum potassium difference was no statistically significant (P>0.05), group comparison, the serum potassium in treatment group showed no significant difference before and after, but there are slightly elevated but within normal range, then it was declined. In control group, no significant difference of serum potassium between groups before and after, but there continues to rise after the situation increased; 3) Creatinine between the two groups showed no significant difference, in the treatment group has a stable and improving trend, but within group there was no significant difference before and after (P>0.05), compared with initial level and the post-test, the difference of the control group was not statistically significant (P>0.05); 4) At the same time in the observation period, the difference of 24-hour urinary potassium quantitative was statistically significant between the groups (P<0.05). Within groups, the treatment group was significantly different between before and after,while the control group, no significant difference between before and after.Conclusion 1)The study did not find Jian Pi Yi Shen Prescription cause the Hyperkalemia of chronic renal failure that symptoms of Spleen and Kidney Qi Deficiency; 2)Oral medicine Jian Pi Yi Shen Prescription can ensuring the efficacy and the same time potassium metabolism can be adjusted to promote the excretion of potassium, reasonable use can ensure the safety; 3)In comparison with the treatment group, Benazepril Hydrochloride in renal failure (CKDâ…¢period) there was no significant difference between hyperkalemia difference; 4) because of some factors such as sample size and observation time, the therapy on Jian Pi Yi Shen Prescription has yet to be made an in-depth study on the long-term impact of renal function and the regulation mechanism of the serum potassium.
