Objective To study the distribution of SNP276 in adiponectin gene in Han people of Shanxi region and its association with components of metabolic syndrome in population of impaired glucose regulation.Methods The study population consisted of 436 Chinese Hans residents in Shanxi province. All of subjects underwent 75 g oral glucose tolerance test (OGTT) was divided into 128 subjects with normal healthy and 308 subjects with impaired glucose regulation (IGR) which was composed of 162 subjects with impaired fasting glucose (IFG) and 146 subjects combined IFG and impaired glucose tolerance (IGT). Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was applied to test the adiponectin SNP276G/T polymorphism. Meanwhile height, weight, waist circumference, systolic blood pressure (SBP) and diastolic blood pressure (DBP) were measured by the professional staff, calculated body mass index (BMI).And tested the serum fasting plasma glucose (FPG) by glucose oxidase method, total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C), C reactive protein (CRP) by immunoturbidimetry. Compare clinical variables between groups using t-test and frequencies among groups by chi-square test. Analyze the association of different genotypes with the clinical variables using analysis of variance. All data were used to process by SPSS 11.3.Results1.Compared with normal healthy control group, the levels of BMI,waist circumference, SBP, DBP, FPG, TC, TG, LDL-C and CRP were higher in IGR experimental group, which displayed significant difference(P<0.05). While the level of HDL-C was lower, there was no difference (P>0.05). In addition, the detection rates of metabolic syndrome in IFG groups and IFG+IGT groups respectively were 35.2% and 67.8%.And there was significant difference between the two groups.2.The genotypes of APM1 SNP276 were 0.453 GG,0.398 GT and 0.1148 TT and the major allele was G (frequency=0.652) in subjects with normal healthy control group. The distributions of genotypes and alleles of SNP276 both displayed significant difference between normal healthy control groups and IGR groups (P=0.005 and 0.001).The SNP276 non-TT (GT+GG) genotype was associated with increased risk of IGR (OR=2.063,95%CI:1.086-3.916,P=0.027).3.The levels of BMI, waist circumference, CRP and TG in normal healthy control groups with SNP276 GG, GT and TT displayed significant difference (P<0.05).Subjects with GG genotype had higher levels of BMI, waist circumference, TG and CRP than did those with TT genotype, and there was significant difference (P<0.05). In IFG group, the subjects with SNP276 GG or GT genotype had higher levels of BMI, waist circumference,CRP than did those with TT genotype, and there was difference (P<0.05). In IFG+IGT group, there was significant difference in the levels of BMI, waist circumference, CRP, SBP, DBP, FPG, TG with the three genotypes (P<0.05).And SNP276 GG carriers had increased the levels of BMI, waist circumference, CRP, SBP, DBP, TC, TG when compared with TT genotype, and there was significant difference (P<0.05).4. In IGR group, the distribution of the subjects complicating components of metabolic syndrome among the three genotypes:the number of the subjects complicating center obesity was 175,accounting for 56.8% in groups with IGR. And the number of subjects with the three genotypes of APM1 SNP276 was 115(65.7%) GG,55(31.4%) GT and 5(2.9%) TT, and there was significant difference (P=0.000).The SNP276 non-TT (GT+GG) genotype was associated with increased risk of center obesity (OR=5.667,95%CI:2.057-15.610,P=0.001).There were 232 subjects combining with abnormal lipid metabolism, accounting for 75.3%,of which there were 187 cases(80.2%) with hypertriglyceridemia. And the number of those subjects with the three genotypes of APM1 SNP276 was 148(63.8%) GG,71(30.6%) GT and 13(5.6%) TT, which displayed significant difference (P=0.000). The subjects with (GT+GG) genotype had higher risk to have abnormal lipid metabolism(OR=2.857,95%CI:1.219-6.666,P=0.016). There were 118 subjects combining with hypertension, accounting for 38.3%,of which with the three genotypes of APM1 SNP276 was 77(65.3%)GG,37(31.4%) GT and 4(3.4%) TT, and there was significant difference (P=0.028). The subjects with (GT+GG) genotype increased the risk of being hypertension(OR=3.353,95%CI:1.117-10.066,P=0.031).Conclusion1.In population of impaired glucose regulation, people more liability complicated metabolic syndrome, and the risk of developing to be diabetes and cardiovascular diseases increased.2.In all groups, the variables about metabolism in G carriers increased, while HDL-C declined. The frequence of allele G increased in groups with IGR. Compared to subjects with TT genotype, the detection of the components of metabolic syndrome was higher in G carriers of IGR.3.SNP276 in APM1 was associated with the susceptibility to be with IGR in Han people of Shanxi region, and increasd the occurrence of central obesity, hypertension and abnormal lipid metabolism. Allele G was a risk factor of diabetes, metabolic syndrome and cardiovascular disease.
