BackgroundCurrently, the clinical classification of stroke syndrome, are main rely on "look, smell, and asked, palpation "four clinics as the basis of data collected, the lack of modern science, objective basis of indicators, and the dialectical process is also a mix of personal subjective opinion, so the accuracy and objectivity of diagnosis and treatment biased the inevitable tendency, with a lot of blindness.Acute stroke of yin-yang dialectical system is a large number of Chinese workers in contemporary summary of previous experience in a large number of clinical trials and obtained by the more objective, quantifiable, the dialectical system; and modern inspection methods transcranial Doppler ultrasound combination of traditional Chinese medicine diagnosis so as to providing the objective quantitative, material basis of disease prognosis material, and provide an objective basis to judge prognosis of the disease.object iveTO explore the relationship between intracranial hemodynamic changes after Yin-Yang syndrome, NIHSS scores, prognosis of acute ischemic stroke patients, to test the results of TCD can assist to the ischemia stroke macro differentiation, illness and prognosis assessment. MethodsA retrospective study approach to investigate patients with acute ischemic stroke,849 cases, selected 320 cases met the inclusion criteria of the study. Record general information, yin-yang syndrome, admission NIHSS score, length of stay, head MR findings, and TCD findings. And under the guidelines of 2009 TCD ultrasound diagnostic criteria, lesion side vascular blood flow velocity is divided into groups to speed blood flow group, the normal blood, and slow down group. Took the blood flow velocity in focus intracranial arteries as dependent variable, had one-way factor analysis with the formerly history, yin-yang syndrome type of TCM, NIHSS score,SIP and Complications. Based on the TCD results, patients were divided into a collateral circulation group, no collateral circulation group were compared NIHSS scores of patients admitted to hospital and short-term prognosisResult1.320 patients met the criteria of the middle cerebral artery ischemic stroke patients was included,61 patients with fasted blood flow velocity (19.1%), blood flow slows down 72 patients (20.5%),187 patients with normal blood flow velocity (58.4%); Yang class of 100 Cases (31.25%), yin type certificate (68.75%). Open a collateral circulation in 16 cases (5%)2. Univariate analysis Factors that class certificate of yin-yang syndrome associated indicated that:the distribution of yin-yang syndrome were related to Smoking, drinking, sex:Male, previous smoking history, drinking history patients with positive higher ratio of patients with yang-syndrome.3.The relationship between yin-yang syndrome and prognosis:The admission NIHSS score, discharge NIHSS score, NIHSS improved score, length of hospital stayed were different between yin type syndrome and yang type syndrome: Yang-syndrome with higher admission NIHSS score, discharge NIHSS score, NIHSS improve score length of hospital stay than the yin-syndrome.4. Single factor analysis blood flow velocity change related with the MCA supply area cerebral infarction patients:age, yin-yang syndeome were different among the blood flow velocity slowed, normal, fasted groups, the speed5. The relationship between blood flow velocity changes and the prognosis.(1)Blood flow velocity slowed and fasted groups were different with the blood flow velocity normal group in the admission NIHSS score, discharge NIHSS score:Blood flow velocity fasted groups have an higher ratio of yang-syndrome, Blood flow velocity slowed groups have an higher ratio of yin-syndrome.(2) Blood flow velocity fasted groups were different with the blood flow velocity slowed and normal group in progressive stroke:Incidence of SIP was higher in the blood flow velocity fasted groups than slowed and normal groups. (3) Blood flow velocity greater than 160cm/s and the flow velocity is less than 160cm/s were different in the number of days of hospitalization, the incidence of SIP, the patients with lower dVACA/nVACA have an higher admission NIHSS score than the normal and increase patients.ConclusionThe changed in blood flow velocity that tested by TCD was correlativity exists with Yin-Yang syndrome, state of the illness, prognosis. Performance of the fasted group appeared more in the Yang type syndrome, normal and slowed group appeared more in the Yin type syndrome; The fasted group on admission and discharge NIHSS score higher than normal and slowed blood flow velocity group; The fasted group with progressive stroke incidence, the average hospital stay were higher than the blood flow slowed down and normal group, the patients with lower dVACA/nVACA have an higher admission NIHSS score than the normal and increase patients.
