The worldwide high incidence of coronary heart disease angina pectoris has made it a great enemy of human health. Under the employment of multiple modern western medicine methods, life expectancy of angina pectoris patients has been significantly prolonged and angina pectoris had become a chronic disease. TCM, instructed by the holistic view of human body, could see diseases at a developmental perspective. Its view of prevent before the onset of diseases, prevent diseases from progressing, and prevent second attack after recovery is accord with the view of modern chronic diseases management. Besides, TCM is scientific and contemporarily adaptive. TCM methods are convenient, effective, and inexpensive. So TCM is considerable on the treatment and prevention of angina pectoris. From ancient times to today, the TCM basic understanding of angina pectoris has becoming complete. Modern lifestyle has changed from the ancient times and from the Clinical and research results, we found that blood stasis and heat stagnation syndrome exist in some of angina pectoris patients. Based on his clinical experience of years, My master advisor, prof. Wang Jie treat CHD angina pectoris with blood stasis and heat stagnation syndrome by clearing heat and detoxifying, activating blood circulation and supplementing qi, and relieving stagnant heat. This method is fairly effective in clinic.ObjectiveTo evaluate the effect and safety of activating blood circulation, clearing heat, and supplementing qi Decoction on patients of CHD angina pectoris with blood stasis and heat stagnation syndrome.Methods60 cases of patients of CHD angina pectoris with blood stasis and heat stagnation syndrome were recruited from cardiovascular department of Guang’anmen hospital, China academy of Chinese medical science. Patients were assigned into treatment group and control group according to random digit table,30 patients of each. Patients in treatment group were treated with the decoction (1 dose bid.) and routine western medicine, while patients in control group were treated with placebo (1 dose bid) and routine western medicine. The treatment course was 4 weeks. The angina attack frequency and duration, Nitroglycerin Dosage,12 lead ECG, TCM symptoms score, hs-CRP, TC, TG, HDL-C, LDL-C, PT, APTT, FIB, TT, ALT, AST, BUN, Cr and other safety indicators were observed before and after the treatment.ResultsOn the effective rate of Angina symptoms, treatment group(76.7%) was significantly higher than the control group(40%,p=0.024). Compared with control group the Angina attack frequency and duration of treatment group was significantly decreased(p=0.018, p=0.014). Two groups shows no significant difference on the ECG effective rate and nitroglycerin stop and reduction rate(p=0.787, p=0.114). On the TCM symptoms score, compare with control group, the treatment group significantly decreased (p=0.028). On specific TCM symptom effect, treatment group shows significant effect on the release of chest distress and palpitation(p=0.037, p=0.049). On the laboratory indicators, the hs-CRP level of treatment group was significantly lowered compared with the control group (p=0.006). The TC level and TT level of two groups before treatment is significantly different, so we did not analyze is two indicators. The TG, HDL-C, and LDL-C level show no significant difference between two groups(p=0.943、p=0.083、p=0.720). And the PT, APTT, and FIB level also show no significant difference between two groups. During the treatment course, no obvious adverse reaction and cardiovascular event happened on all the patients in both group. And vital signs(temperature, breathe, heart rate, blood pressure), hepatic and renal function, Blood routine examination, urine routine examination, stool routine examination of all the patients in both group are all normal.ConclusionThe activating blood circulation, clearing heat, and supplementing qi decoction is effective on the clinical symptoms of patients of CHD angina pectoris with blood stasis and heat stagnation syndrome. It helps resist myocardial ischemia and cardiovascular inflammation reaction. It is effective and safety and is worthy of being widely applied in clinic.
