Objectives; To study the relationship between the early T wave inversion in patients with first acute ST segment elevate myocardial infarction(STEMI) after direct percutaneous coronary intervention(PCI) and the plasma brain natriuretic peptide (BNP)levels. It is the clinical useful value in the first acute ST segment elevate myocardial infarction(STEMI) after direct percutaneous coronary intervention(PCI).Methods:Continuous collected of 74 cases with first STEMI after the direct percutaneous coronary intervention(PCI) as the experimental objectives, underwent 12 or 18-lead ECG and measured serum levels on hour 0,6,12 and 24 were classified into two groups according to the recording:pattern A-inversion T waves, pattern B-uninversion T wave. Observing cases' cardiac dysfunction at the same hours.Measured brain natriuretic peptide (BNP) concentrations by enzyme-linked immunosorbent. Compared BNP levels at the A-groups and the B-groups。we measured the left ventricular ejection fraction(LVEF) by the Echocariogram, We estimated relationship of 12 hour's T wave inversion and 7day's left ventricular function. after the direct percutaneous coronary intervention(PCI).Results:1.The BNP of A and B-group had no significant difference (P>0.05) on hour 0> 6 after the PCI,the difference had not statistical significance. The BNP of A and B-group had significant difference (P<0.05) on hour 12,24 after the PCI, the difference had statistical significance.2. The cases of the cardiac dysfunction of A and B-group had no significant difference (P>0.05) on hour 0,6 after the PCI, the difference had not statistical significance. The cases of the cardiac dysfunction of A and B-group had significant difference (P<0.05)on hour 12,24 after the PCI, the difference had statistical significance. The left ventricular ejection fraction (LVEF) of A and B-group on day 7 had significant difference (P<0.05) on hour 12 after the PCI, the difference had statistical significance. Conclusion:BNP predicts cardiac dysfunction. The BNP of A and B-group had significant difference on hour 12,24 after the PCI, at same time, the cases of the cardiac dysfunction of A and B-group had significant difference. the T wave inversion on 12 hour after PCI may be a simple, useful indictor which predicts better cardiac function.
