Research On Relationship Between Weaning And Respiratory Mechanics Change During Smartcare / Ps

Objective By investigating the indicators of respiratory mechanics between groups of success and failure during the Smartcare (SC) weaning progress, to approach the guide significance of SC to weaning and analyze the fiting crowd and the main affecting factors of Smartcare (SC) .Methods According to the SC weaning prompts and the the results of actual weaning state within 2 hours, 76 cases of critically ill patients were divided into: the group that SC weaning prompted success and the actual weaning was successful (that is the real success), the group that SC weaning prompted success and the actual weaning was failure group (that is the false success), the group that SC weaning prompted failure and actual weaning was also failure group (that is the real failure). Comparing respiratory rate (RR), tidal volume (Vt), minute ventilation (MV), end exhaled carbon dioxide (etCO2), dynamic compliance (Cdyn), airway resistance (Raw), maximum inspiratory negative pressure (MIP), mouth occlusion pressure (P0.1) of each group during the SC weaning process.Comparing rapid shallow breathing index (RSBI) of each group in the weaning for 15 minutes, 30 minutes, 45 minutes, 60 minutes time period.Analyzed the incidence rate of all kinds of primary disease in False positive group .And conduct comparison of weaning affect factors in successful group and failure group.Results The Cdyn indicators of the false success group,the real failure group patients were worse than the real success group, RR indicators were faster than the real success group , Vt indicators were less than the real success group , MIP indicators were worse than the real success group,existed statistical significance.The RSBI of the false success group, the real failure group in 15 minutes, 30 minutes, 45 minutes, 60-minute period was significantly higher than the real success group (P<0.05). Nerve, muscle diseases was the highest incidence in the false success group. The comparison of APACHEâ…¡score, mechanical ventilation time of the false success group and the real failure group to the real success group's existed statistical significance.Conclusion There are some phenomenon of false success for Smartcare / PS guiding clinical weaning,false positive rate is 15.8%. Compared with the successful SC Weaning patients, Cdyn, RR, Vt, MIP and other respiratory mechanics indicators in SC failure patients had poorer performance. The dynamic change of RSBI and MIP have more accrute a guide significance in SC weaning.The main factors that affect the weaning were higher APACHEâ…¡score and prolonged mechanical ventilation.
