Clinical Investigation Of Fentanyl After The Administration Of Dezocine

Objective:To evaluate the analgesic value of dezocine, and to estimate if it can reduce the dose of fentainyl patient-controlled intravenous analgesia (PCIA)and (or) enhance the analgesic function and restrain the side effects.Methods:30 patients(36-75 years old, ASAⅠ-Ⅱ) undergoing higher abdominal surgery were randomly divided into 3 groups:group A, B and C,each induced 10 cases. We introduced the usage and emphasis of PCIA unit, explained the VAS methods to patients and measured VC VT of the patients at our preoperative visit time.The model of PCIA was loading dose 4ml, background infusion dose 1.5ml/h and bolus dose 3.5ml with lock time 10 minutes and the maximum dose was 100ml. PCIA was performed as soon as the operation was finished. The combination agents for PCIA in these groups were as follows:In group A, dezocine25mg+maxolon 10mg; group B, fentanyl lmmg+ maxolon 10mg; and group C, fentanyl 0.8mg+dezocinel0mg+ maxolon 10mg. We then visited the patients at 1,4,8,24 and 48 hours after operation, recorded the blood pressure, heart rate, breath rate and pulse oxygen saturation(SPO2), side effects(nausea, vomiting and breath restrain) and the recovery condition of intestines'function were evaluated, too. We adopted VAS, calm score and recoded the patients to pressed PCIA times in 48 hour and measured VC VT of the patients after operation finished 12th.Results:(1)There was no significant difference of the blood pressure, heart rate, breath rate at 1,4,8,24 and 48 hours after operation(P>0.05).(2)The pulse oxygen saturation(SPO2) group B was lower than group A and C(P<0.05). (3) The patients'assessment to postoperative analgesia:In group A had one patients who felt severe pain, all the others were satisfied with analgesia. There were no differences on VAPS at all the time points(P>0.05). (4) There were differences on calm score in 3 group, group A higher than group B and C (P<0.05).(5) The 48 hour of press PCIA and at all the time points in group C was markedly less than group B and C (P<0.05), and group B was significant difference than group A(P<0.01).(6) There were no differences on the VT of the patients before and after operation finished 12th change,but zhe VC group C was markedly less than group B and C (P<0.01).(7) Three patients in group B felt nauseant(30%) and one of them had a vomit. In groupC had a patient who felt nauseant(10%). Two patients in group A and B, one in group C were somnolent.(8) There were no differences among all the patients on the recovery duration of intestines'function.Conclusion:Dezocine can alleviate zhe postoperative pain.Combine dezocine and fentanyl PCIA can obviously reduce zhe postoperative pain,it also can enhance the efficiency of analgesia and restrain the side effects, too.
