Electric Acupuncture Treatment Of Post-stroke Anxiety Disorders Of Follow-up Study

[Research background] In China, due to the high incidence rate of cardiovascular and cerebrovascular diseases, the incidence rate of anxiety disorder after stroke was also increased year by year. In recent years, post-stroke emotional disorders become to the research hotspot among neurology doctors, but there are few reports about anxiety disorder after a stroke, the reports of acupuncture comparative study are even less, the research about tracking follow-up study on treatment of PS AD is almost blank. Since the many side effects of the anxiolytic western medicine, the expensiveness and Oriental stigma, the drug contradiction has not been resolved. The acupuncture treatment shows the characteristics of an economic, convenient, safe, effective and better compliance. In principle, if both the drug treatment and non-drug may be effective in patients with anxiety, the non-drug treatment should be preferred. Early attention and having treatment of PS AD not only help to improve the quality of life of patients and their functional recovery, but also to reduce morbidity and mortality. Therefore, further study of PS AD about the efficacy of acupuncture and western medicine treatment and anxiety symptoms trend after six months has important clinical significance.[Research objective] This research has object of patients who have both stroke disease and anxiety disorder afterwards. We formulate rigorous criteria on diagnosis, inclusion and exclusion, and select guidelines recommended first-line treatment SSRI paroxetine as a standard control. The selected evaluation indicators are internationally recognized indicators, through a strict and randomized controlled study, a comprehensive and objective evaluation about the efficacy of electro-acupuncture treatment for PSAD is carried out. And also, we observed the trend of anxiety symptoms of the patients in the next six months to provide reliable clinical evidence for the effect of acupuncture treatment of this disease.[Method] Object:All the cases were from the China Academy of Chinese Medical Sciences, Guang An Men Hospital acupuncture inpatient wards, the patients who were diagnosed as anxiety disorder after stroke. The acquisition time of cases started from March2011to October2011. The patients were enrolled strictly in accordance with the standards of inclusion, and were divided into treatment group (acupuncture group) and control groups (western medicine group) by using the random number table. Both of the2groups were treated for4weeks, which acupuncture group was treated with acupuncture and electro-acupuncture, and western medicine group received standardized treatment with oral paroxetine hydrochloride. The HAMA and SAS scales were graded at enrollment,1week,2nd week,4th week on PSAD patients. By analyzing the HAMA reduction fraction and the HAMA factors, we evaluated the treatment efficacy of the2groups. And the patients were tracked after3months and6months of the treatment for follow-up study on their trend of anxiety symptoms.[Result] Total effective rate after treatment in Western medicine group is86.95%,and85.71%in acupuncture group. The effective rate between groups of Western medicine and acupuncture shows no significant difference after X2test. Results of factor analysis:comparison between2groups:after treatment, the factor of HAMA analysis showed that mental anxiety factor in Western medicine group showed significant differences with acupuncture groups; Somatic anxiety factor, acupuncture group showed significant differences with Western medicine groups; Sleep factor in two comparison groups were not significantly different. Follow-up study:2groups of patients after treatment of3months and6months showed no significant difference between the groups in the group. Incidence of adverse reactions: in the research process,6cases showed undesirable reactions in the Western Group, of all group accounts for26.1%; no adverse reaction occurs in acupuncture group.[Conclusion] Both of acupuncture group and western medicine group of anxiety symptoms after treatment can be achieved with satisfactory results, and the effects were equivalent. The better improvement of self-care ability is, the better anxiety symptom improves. The western medicine treatment has better effects on improving the mental symptoms, but the acupuncture group has advantage of improving the physical symptoms. The anxiety symptoms of both groups in6months were relatively stable. The acupuncture group has more rapid effectiveness, less side-effect and higher therapeutic index than western medicine group.
