Objective:To understand the current status treatment of convulsive epilepsy in rural areasof Jilin Province and effect of Phenobarbital and evaporate, adverse reactions. We canfurther discuss how to effectively control of seizures in less developed areas, how touse antiepileptic drugs regularly and reduce the cost of treatment as far as possible inorder to reduce the patient and family’s psychological and economic burden and wealso aim to reduce the mortality of patients with epilepsy and improve their quality oflife.Methods:Selecte seven cities/counties (Panshi, Huadian, Dongfeng, Meihekou, Tonghua,Dunhua, Fuyu, Huinan, Taonan) in Jilin province to carry out epilepsy preventionmanagement project in Chinese rural areas. Trained township hospital doctors conducta preliminary screening from the patients who are confirmed or suspected convulsiveepilepsy. Then a neurologist who is in charge of the project review decides thepatients whether they will go to the treatment management group and getPhenobarbital for free, if it doesnot work, give the patients sodium evaporate for free.After that the patients will be followed up regularly and fill out the follow-up table.The doctor will use the software EpiData3.1and SPSS17.0to record and analysisdata. So that the effect and adverse reactions of Phenobarbital, evaporate in thetreatment of Jilin Province’s epilepsy can be understood.Results:2,192patients were selected, including1,234men (56.3%) and958women(43.7%). The ratio between men and women was1.29:1. The average age is41.13±15.187, The0-20years old epilepsy patients accounted for about56.6%Therewere1,080active epilepsy patients which accounted for about83.1%and thetreatment gap accounted for40.72%. Not regular treatments include traditionalChinese medicine, acupuncture, catgut embedding therapy, cut and folk recipe. The most common type of attacking is generalized tonic, clonic seizures which accountedfor about72.8%. There are1218epileptic patients in phenobarbital treated group.There are165epileptic patients in valproic acid treated group. There are222epilepticpatients quit the group. The most common reason of quitting the group is the familyagainst. They disagree to take the medicine or the effect is not good. There were30epileptic patients dead. The most common reason of death is cerebral vascular diseasewhich accounted for30%. Phenobarbital mild adverse reactions may be drowsiness,sleepiness, dizziness and headache. Take the medicine for a full year, significant ratewas71.3%, effective rate was4.9%, total effective rate was76.2%. Multivariateanalysis showed that Phenobarbital treatment is a protective factor for the finaloutcome and the strength of the correlation value of OR was0.376(95%C.I.0.216-0.655). The common adverse reactions of VPA are headache, drowsiness,fatigue, nausea and vomiting. The total efficiency rate will be92.2%after a year. Dueto the sample size was small, multivariate analysis did not display significant factor.Conclusion:The ratio of epilepsy patients between men and women were1.29:1. Childrenand adolescents has higher incidence than adults. Not regular treatments includeTraditional Chinese medicine, acupuncture, catgut embedding therapy, cut and folkrecipe. After the systematic and formal project management of rural epilepsy, theepilepsy’s treatment status of Jilin Province has improved now. Phenobarbital,evaporate are well tolerated among the most people. There are fewer adverse reactionsin patients. Long-term use will be safer. Using Phenobarbital and evaporate to treatconvulsive epilepsy has a good effect, a small dose epilepsy can get better controleffect. The most important factor affecting the efficacy of antiepileptic drugs is formaltreatment.
