Background:Acute pancreatitis (AP) is a kind of complicated mechanisms, acute abdomen in serious condition, according to the severity of the disease were divided into mild acute pancreatitis (MAP) and severe acute pancreatitis (SAP). Light good prognosis, most are self limiting, SAP mortality rate is10%-25%, has not yet been completely elucidated the mechanism. Since Bone proposed the systemic inflammatory response syndrome and compensatory anti-inflammatory response syndrome hypothesis, research about AP and proinflammatory and anti-inflammatory cytokines balance began to appear.Objective:By detecting the AP serum proinflammatory cytokine interleukin-6(IL-6) and the anti-inflammatory cytokine interleukin-10(IL-10) level, and combined with rehabilitation period patient, understanding the change and significance of anti-inflammatory and proinflammatory cytokine levels in the course of the disease, to explore its effects on prognosis prediction.Methods:The onset time of no more than a day of AP patients and healthy people as the research object, according to organ dysfunction or local complications, APACHE II score and pancreatic CT classification as grouping criteria, divided into SAP group, MAP group and control group, In group SAP after admission,3,7,10, first,14day fasting peripheral blood samples, for the detection of serum IL-6and IL-10levels; group MAP patients because of the recovery period and a shorter hospital stay, admission to the hospital after1,3,7,10days of fasting from peripheral analyzed the detection. The two group and healthy controls were recorded, and compared with each otherResults:Mild acute pancreatitis in patients in group IL-6, the first day of admission levels higher than the control group, after3,7,10day level continued to decline, until there was no significant difference with control group. The SAP group was significantly higher than that of IL-6were mild acute pancreatitis group, and continued to rise after admission, until3-7days after hospitalization could be detected obviously decline, to fourteenth days with the MAP group and the control group were not statistically significant. Group SAP and group MAP IL-10levels were from the first day to the hospital was significantly higher than the control group, and MAP group and SAP group have significant differences, with the prognosis of the disease, is to narrow the gap between the two groups and no statistical significance, while group MAP rebound fell to significantly lower than the control group, there is statistical significanceConclusion:To promote the dynamic balance between inflammatory and anti-inflammatory cytokines plays an important role in the occurrence and development of AP duration, the role of the two results to decide the outcome of treatment of pancreatitis, focus on the SAP is the anti-inflammatory and proinflammatory cytokine balance at low level, therefore, strengthen the inspection of the two party, especially detection of IL-6and IL-10, has the vital significance.
